Student/Family Waivers & Policies

I,hereby release Maralee’s Dance Productions & Design, LLC, its owners, directors, instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Maralee’s Dance Productions & Design, LLC. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families

I release Maralee’s Dance Productions & Design, LLC, its owners, instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.

TUITION AGREEMENT

I understand Tuition is month to month fees to be paid by first of the each month, or set up on automatic payments (choice of 5th, 15th or 20th) of each month or a $10 late fee per month may be charge for unpaid tuition, and/or a $10.00 late fee for any unpaid costumes after due date. If arrangements or a payment is not received in a period of 2 months then the account may be handed over to a collection agency.

CLASS WITHDRAWAL AND DISCONTINUE AUTOPAY

I understand I will need to write/send a letter of request to withdraw & to stop future automatic payments, requesting that Maralee’s Dance Productions & Design, LLC from making future automatic payments, at which time any unpaid charges on my account will be authorized to finalize my account balance.
I understand that there are NO refunds for registration fees, costume fees, recital fees, dancewear fees, any class tuition or fundraiser credits I may have acquired once my child has participated at Maralee's Dance Productions & Design, LLC.

STUDIO PORTRAITS, PERFORMANCES & CLASS VIDEOS/PHOTO PERMISSIONS:\

I give permission to Maralee’s Dance Productions & Design, LLC to use any pictures and or videos of my child for advertising purposes in print, any promotional videos and on the website.

PARTICIPATION AGREEMENT:

I have read via rates/policies link to Maralee's Dance Production & Design website for general policy information and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Maralee's Dance Production & Design does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Maralee's Dance Production & Design, and I voluntarily assume the risks associated with such participation.

ELECTRONIC SIGNATURE AGREEMENT:
By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
