CLASS BEGINS

Payment Policy:
Monthly tuition payments are due the first of each month regardless of any absences. Tuition is split between 9 months, September – May regardless of any cancellations due to weather, holiday breaks, recitals, or any other unforeseen circumstance. Tuition is non
refundable and will still be rendered for students on injury probation. There will be a 10 day grace period so a $25 late fee will be
applied to any tuition payment received after the 10th day of the month. Once competitions, costumes, shoes, tights or any other accessory are paid for there will not be refunds given. If I choose to participate in a fundraiser conducted by Optimum Dance Complex I understand that the money is to be used for any event or item organized by Optimum Dance Complex and not for personal use outside of the studio. I understand that money in my fundraiser account is never to be given to me as cash. If I do not to return to Optimum Dance Complex for any reason, all funds in my fundraising account will be forfeited to the Optimum Dance Complex general fundraising account. Payments accepted are cash and check only.

Media Policy:
I grant Optimum Dance Complex by Gabrielle Durci and its affiliates, successors and assignees, and any person acting under their permission and authority the unqualified right, privilege and permission to use photographs, videos, slides, transparencies and recordings of voice or any other image may used to illustration types of media. I further acknowledge that I have no interest whatsoever in any professional fees derived by Optimum Dance Complex by Gabrielle Durci from the subsequent performance of professional services for the person or persons who view my photographs. I do not expect compensation, financial or otherwise, for the use of these photographs.

Injury Release Waiver Optimum Dance Complex by Gabrielle Durci Liability Waiver and Acknowledgment of Risk:
I understand and agree that participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any Optimum Dance Complex by Gabrielle Durci classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Optimum Dance Complex by Gabrielle Durci, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, death to my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Optimum Dance Complex by Gabrielle Durci. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Optimum Dance Complex by Gabrielle Durci, its owners, agents, volunteers, assistants,
employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
