Release and Waiver of Liability and Protection Agreement-BROADWAY NORTH

Physical activities such as dance and acro carry a risk of injury to students. I hereby release Broadway North Dance Studio of any liability whatsoever arising out of an injury, damage or loss which may be sustained by said person during the course of involvement with Broadway North Dance Studio. This includes classes held in the dance studio and during any other performance or rehearsal with Broadway North. I have read this waiver agreement, understand its contents, and accept and assume such risks. Please sign and date in the box below:

Video/Photo Release - Broadway North is hereby granted permission to take photo/video of dancer(s) to use for social media, brochures, website, and advertising purposes for the Broadway North School.

• Enrollment is for September 2024 – June 2025. There must (10) payments for the year. ANY changes in enrollment must be submitted in writing. We will hold you/your child’s place in the class until such notice is given. If you/your child misses’ classes without providing a change in enrollment you will be responsible for any charges to the account up to the day such notice/documentation is received – It is YOUR responsibility to keep us informed… we have almost 300 students and cannot “track” each individual. There are no refunds on tuition or for missed classes.

• Tuition payments are due by the 1st of each month. Any balances on the account after the 15th of the month will be charged a $20.00 late fee. BALANCES CAN NOT EXCEED $150.00 TO CONTINUE WITH DANCE. Billing discrepancies need to be addressed within 60 days from date incurred.

• Costume charges are in addition to tuition charges and will be paid in advance of receiving the costume. A costume deposit of $50.00 is due with your October tuition. We will be measuring students in October, please make sure your student is measured before Christmas Break. Costumes are ordered early to ensure a good selection; the full cost is your responsibility – whether the student continues w/the class or not – costumes are non-returnable/non-refundable.
