LIABILITY WAIVER
In consideration of being allowed to participate in dance classes, workshops, and activities offered by Doxa Dance Ministry, located at 3801 S. Western Amarillo TX 79109, I, the undersigned participant (or parent/legal guardian if participant is a minor), hereby acknowledge and agree to the following:
Assumption of Risk: I understand and acknowledge that dance activities involve physical exertion and can be strenuous. I voluntarily assume all risks associated with participation, including but not limited to falls, contact with other participants, and any injuries arising from my own actions or the actions of others.
Release of Liability: I release and discharge Doxa Dance Ministry, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in dance classes or events provided by the studio, including those allegedly attributed to the negligent acts or omissions of the above-mentioned parties.
Indemnification: I agree to indemnify and hold harmless Doxa Dance Ministry and its representatives against any and all claims, suits, or actions of any kind whatsoever for liability, damages, compensation, or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs, if litigation arises pursuant to any claims made by me or by anyone else acting on my behalf.
Medical Authorization: In the event of an injury or medical emergency, I authorize Doxa Dance Ministry to obtain medical treatment for me, including transportation to a medical facility, and I agree to be financially responsible for any medical bills incurred.
Photography and Publicity: I understand that Doxa Dance Ministry may take photographs or video of classes, rehearsals, performances, camps and workshops, or other ministry events for use in website, social media, print, or other media according to the ministries and purposes of Doxa Dance Ministry. Names of dancers are not included with the photograph without express permission given by a parent for the specific photo in question. I understand I can deny permission to use photographs or video of my child according to the aforementioned purposes by checking the box under Waivers --> Photo Opt-Out in my parent portal account. I can change this setting at any time.
Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflicts of law principles.
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
