Photo & Liability Release & Indemnification:
Above named dancer (hereandafter student) has my permission to attend Dancers' Corner for dance instruction, dance education, and associated activities. Student also has my permission to use all facilities and equipment at Dancers' Corner and to participate in all dance routines and training. I hereby confirm that I have authority to sign this release on behalf of myself, student, and others for whom I am responsible. I confirm that student is in good health. I also confirm that student has no limitation in the physical activities, training, dance instruction or associated programs to be provided by Dancers' Corner. I also agree that in the event any such limitations are required in the future, I will promptly notify Dancers' Corner about the limitation. I authorize Dancers' Corner to provide minor first aid such as band-aids, application of an ice pack, or other minor treatment. In the event that I am unavailable, I also give Dancers' Corner, its owners, directors, officers, agents, employees, contractors, instructors, attendants, assistants, volunteers, and assigns permission to obtain emergency medical treatment for student if it is needed. I have been fully informed by Dancers' Corner via this release form and orally about physical demands of the dance class and program at Dancers' Corner. I also acknowledge that if I had any question about the physical activity and the dance program of Dancers' Corner, prior to my execution of this release, Dancers' Corner staff was available to answer my questions. By executing this release and indemnification, I agree I had no unanswered questions. I fully understand that any activity involving motion, including the activities for which student is enrolled, create the possibility of serious injury or death just like other exercise or physical activity. I fully understand and assume this risk on behalf of student and myself. I acknowledge that this risk can be significantly reduced by properly following Dancers' Corner's rules and policies. I also understand that it is not Dancers' Corner's responsibility to prevent a violation of its rules and policies.
In consideration for the services received and the risks assumed by me on my own behalf, and that of student, except for intentional wrongful acts, Dancers' Corner and its owners, directors, officers, agents, employees, contractors, instructors, attendants, assistants, volunteers, and assigns are fully and absolutely released and discharged from all claims for injury, disability, disease, damage, loss and expense (past, present, and future) arising out of or connected in any manner with any accident, injury, or claim made by myself, student, others for whom I am responsible, and our heirs, executors, assigns, and representatives. This release applies to all activity in which student is engaged that is in any way directed, sponsored, conducted, or maintained at or by Dancers' Corner, its owners, directors, officers, agents, employees, contractors, instructors, attendants, assistants, volunteers, and assigns. This release is executed of my own free will as consideration for access to the services, facilities, equipment, and dance instruction by Dancers' Corner. I further agree that this release also applies to any other family member or person I have responsibility for while at Dancers' Corner's facilities. I agree to indemnify Dancers' Corner and its owners, directors, officers, agents, employees, contractors, instructors, attendants, assistants, volunteers, and assigns for any cost of defense and claims made by myself, student, and others I have responsibility for as set forth in this agreement.
