Consent and Liability Release:

As the legal parent or guardian, I hereby consent to the enrollment of my child as a student of the Supernova Dance Company LLC, and agree that such enrollment shall be subject to the rules and regulations established by Supernova Dance Company LLC. Supernova Dance Company LLC reserves the right at any time to terminate enrollment of any student who fails to comply with such rules and regulations. In the event of cancelled enrollment, any deposit and tuition shall be forfeited.

 The undersigned hereby agrees to indemnify and hold harmless Supernova Dance Company LLC, it's agents or employees, heirs, successors and assigns, against any liability, claim or expense including attorney's fees and costs, arising out of any claim or suit with respect to the actions of students while a student at Supernova or any injuries or claims occurring during the study of dance at Supernova.

 The Consent and Release is given by the undersigned in partial consideration for the enrollment and acceptance of the student at Supernova and is entered into freely, voluntarily and after the full opportunity for review by the undersigned.
I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury, death, and exposure to Covid-19. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Supernova Dance Company, LLC, classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Supernova Dance Company, LLC, its owners Alexa Conway and Antonia Vega, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Supernova Dance Company, LLC.
I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Supernova Dance Company, LLC, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.



Withdrawal and Refunds:
Fees are non-refundable. Withdrawal notice must be given to the studio directors in writing via email. 
If a student wishes to withdraw from lessons, there is a penalty in the value of one months tuition due at the time of cancelation, as well as any outstanding balances. Any students who withdraw after March 31st will be required to pay April, May, and June tuition in full at time of cancelation. No refunds will be given. Once the studio directors have been given written notice of withdrawal and the penalty has been paid, class tuition will no longer be charged.
Billing Authorization
:
I understand tuition fees will be posted to my child's account each month from September through June. As the parent/legal guardian of the child, I hereby authorize Supernova Dance Company LLC to auto-debit my child's tuition on the 1st of each month, and authorize Supernova Dance Company LLC to auto-debit any other required fees. The parent is responsible for updating billing information and submitting new banking and/or credit card information if accounts are changed or closed. These updates must be made prior to the auto-pay billing date. I understand that the parent/legal guardian must update their own accounts via their parent portal. I understand a late fee of $20 will be posted to accounts for failed charges.



Costume Fees:

It is understood that all registered students will automatically be registered to participate in the June recital. Costume fees will be posted to dancers accounts in the fall before winter break. All accounts must be paid in full in order for your dancers costume to be ordered. Costume balances, if applicable, will be posted to each dancers account in February. Costume fees are non-refundable even if you choose to withdraw from the program. Supernova Dance Company reserves the right to post late fees to costume balances not paid in full by the due dates. All outstanding tuition balances and/or company fee balances must be paid in full prior to the release of costumes to the parent.


Medical Emergency
:
The undersigned gives permission to Supernova Dance Company LLC, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby give permission for my child to be transported via ambulance to the hospital if first responders determine it is necessary. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.


Extreme Weather or Unexpected Interruption of Classes: Supernova Dance Company does not follow Baltimore and Harford County school closings. Closings will be posted on our Social Media accounts and families will be notified via email. There is one snow day/unexpected closing built into your yearly tuition. If there are additional canceled classes, a make-up day will be scheduled if possible.


Dress Code:
Proper dress code is expected for all classes. Dress code is listed on our website. No refunds will be given for missed classes due to improper attire and/or foot wear. 


Attendance & Lateness:
The studio reserves the right to have students who come more than 10 minutes late to class sit out. Repeated lateness and/or excessive absences may result in termination of lessons. Students who miss three or more classes during the last eight weeks of classes may be removed from their dances from the June Recital. Removal from recital dances is at the full discretion of the teacher and/or company directors.


Parent's Responsibility to be Aware of Dates & Events:
It is the responsibility of the parent or adult student to be aware of all studio activities, such as observation days, closings, fundraisers, recital dates, etc. The studio will post all such notices on the website, Facebook, newsletters, etc. It is the parent's responsibility to regularly check their emails to ensure they are informed and up-to-date on the latest studio information.
Calendar & Spring Performance Dates:
I understand that the recital and dress/tech rehearsals will be scheduled for the beginning/middle of June. Exact scheduling will be available in the Spring. Recital is typically the second weekend of June, unless something beyond our control requires us to change the date.


Photo and Appearance Release:
As a registered student of Supernova Dance Company, LLC, the registration to the studio which is hereby acknowledged, I hereby irrevocably grant to you, your successors, it’s agents, affiliates, licensees and assigns, the non-exclusive right, but not the obligation to use, the name, likeness and voice in connection with and as a part of your Supernova Dance Company, LLC programming (the “Program(s)â€, and in all exploitation, advertising, promotion, publicity, title sequence and ancillary marketing and merchandising campaigns. The Program(s) may be exhibited and exploited by you by any and all methods and means in any and all media (including without limitation the Internet, interactive media) now known or hereafter devised throughout the universe in perpetuity. You may also use my name and likeness, and biographical information pertaining to me in connection of the dance school program(s). I hereby release Supernova Dance Company, LLC (and any of its agents, affiliates, licensees and assigns) (the “Producerâ€) from and against any and all claims, causes of action, suits, costs, liabilities and damages whatsoever that I now or hereafter may have against Producer in connection with any program and the preparation, production and/or any exploitation thereof, or any other use of the Events or of any rights granted to you herein. I further understand that neither Supernova Dance Company LLC nor any third party is under any obligation to exercise any of the foregoing rights, licenses and privileges. (If you do not want your child to be photographed or videotaped, a signed and dated Waiver for Photo/Appearance Release must be on file with the studio prior to the child beginning dance lessons. Members of the Supernova Dance Company competition team cannot "opt out" of of the photo release.)


Travel Release:

It is understood that the minor and signed adult legal guardian(s) traveling under the sponsorship of Supernova Dance Company LLC does (do) hereby agree to indemnify, defend and hold the Supernova Dance Company LLC and Chaperon Designees, and each of them their respective agents, heirs, executors, administrators, successors, and assigns free and harmless from and against any and all claims, demand, damages, actions, suits, debts, liabilities, causes of actions, loss cost and expense arising out of or as a result of the travel to and from the trip and the actual field trip activity, including and without limitation, any and all claims of any third party for injury to person or property or any other negligent or wrongful acts of omission of the minor and their adult guardians traveling with the Supernova Dance Company LLC.


Use of Customer Portal
:
Parents are responsible for updating all information on file with the studio including, but not limited to, the following: contact information, phone number, email address, medical information and billing information.
Covid-19 Protocol
The studio is mask optional and we are operating as "normal." We ask if someone in your dancer's home tests positive, they stay home.
