• I agree to read to follow all of HONEY’S SCHOOL OF DANCE’S Policies and Procedures stated therein.
• HONEY’S SCHOOL OF DANCE must be advised of any participant’s pre-existing and present medical condition(s).
• I agree to allow HONEY’S SCHOOL OF DANCE to photograph my child for promotional purposes. I understand the NO release of personal information, such as name or address, will be made.

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Honey's School of Dance classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Honey's School of Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Honey's School of Dance. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Honey's School of Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

Permission is granted Honey's School of Dance to use photographs of students for publicity purposes.

In consideration of the services of Honey's School of Dance, their agents, owners, officers, volunteers, participants, employees, independent contractors, volunteers, interns, and all other persons or entities acting in any capacity on their behalf (from now on collectively referred to as "HSD"), I hereby agree to release, indemnify, and discharge HSD, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows:

I acknowledge that my or my minors participation in dance training and instruction, including dance, gymnastics, and other various disciplines, entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or third parties. I understand that HSD cannot eliminate such risk without jeopardizing the essential qualities of the activity.

The risks include, among other things: slips and falls; falling from equipment; burns; pinches, scrapes, twists, and jolts that could result in scratches, bruises, sprains, lacerations, fractures, concussions, or even more severe life-threatening hazards; strains, cuts, bruises, muscle soreness, and fractures; musculoskeletal injuries including head, neck, and back; injuries to internal organs; the negligence of other people; my physical condition; and the risk of emotional and psychological injuries or physical damage associated with this activity. Traveling to and from shows, meets, and exhibitions raises the possibility of any manner of transportation accidents. If you or your minor is injured, you or your minor may require medical assistance at your own expense.

Furthermore, HSD employees have difficult jobs to perform. They seek safety, but they are not infallible. They might be unaware of a participant's fitness or abilities. They might misjudge the weather or other environmental conditions. They may give incomplete warnings or instructions, and the equipment being used might malfunction.
I expressly agree and promise to accept and assume all of the risks existing in this activity. I or my minor participate in this activity voluntarily, and I elect to participate or let my minor participate despite the risks.
I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless HSD from any claims, demands, or causes of action that are in any way connected with my or my minors participation in this activity or my use of HSD's equipment or facilities, including any such claims which allege negligent acts or omissions of HSD.
Should HSD or anyone acting on their behalf be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and expenses.
I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I am willing to assume the risk of any medical or physical condition I may have.
In the event that I file a lawsuit against HSD, I agree to do so solely in the state of Florida. I further agree that the substantive law of that state shall apply in that action without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.

By acknowledging this waiver, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against HSD based on any claim from which I have released them herein.

In consideration of my minor being permitted by HSD to participate in its activities and to use its equipment and facilities, I further agree to indemnify and hold harmless HSD from any claims which are brought by, or on behalf of Minor, and which are in any way connected with such use or participation by Minor.

I have had sufficient opportunity to read this entire document. I have read and understood it and agree to be bound by its terms.

Participants of Honey's School of Dance (after this, collectively referred to as "HSD") may be photographed or videographed during classes, performances, rehearsals, or any HSD-related activities. With permission from parents, HSD may use these photos to be included for HSD advertisements, any HSD-affiliated events, HSD's website, and/or HSD-affiliated social media accounts. To ensure privacy, HSD will not include the full names of children. No financial compensation shall be given for the use of any photographs. I acknowledge that HSD may choose not to use my photo at this time but may do so at its discretion at a later date. HSD reserves the right to discontinue the use of images without notice. I consent to and authorize the use by HSD, or anyone authorized by HSD, of any photographs or video footage taken of my child. I have been allowed to ask questions for clarity regarding this form. I have read this agreement and understand and accept its contents.

It is Honey's School of Dance's policy to respect your privacy regarding any information we may collect while operating our website. Accordingly, we have developed this privacy policy in order for you to understand how we collect, use, communicate, disclose and otherwise make use of personal information. We have outlined our privacy policy below.

We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned.
Before or at the time of collecting personal information, we will identify the purposes for which information is being collected.
We will collect and use personal information solely for fulfilling those purposes specified by us and for other ancillary purposes, unless we obtain the consent of the individual concerned or as required by law.
Personal data should be relevant to the purposes for which it is to be used, and, to the extent necessary for those purposes, should be accurate, complete, and up-to-date.
We will protect personal information by using reasonable security safeguards against loss or theft, as well as unauthorized access, disclosure, copying, use or modification.
We will make readily available to customers information about our policies and practices relating to the management of personal information.
We will only retain personal information for as long as necessary for the fulfillment of those purposes.
We are committed to conducting our business in accordance with these principles in order to ensure that the confidentiality of personal information is protected and maintained.
Honey's School of Dance may change this privacy policy from time to time at Honey's School of Dance's sole discretion.

STUDENT AND CAREGIVER HANDBOOK AGREEMENT
By signing below and/or checking the box below, I acknowledge that I have received, read, and understood the Honey’s School of Dance Student and Caregiver Handbook. I agree to comply with the policies, guidelines, and procedures outlined in the handbook to ensure a positive and productive dance experience for my child and our dance community. I understand that the handbook may be updated periodically, and I will stay informed of any revisions.
As a caregiver of a student at Honey’s School of Dance, I agree to the following:
1. Regular Attendance: I will ensure that my child attends all scheduled dance classes, rehearsals, and performances promptly and consistently, except in the case of illness or other unavoidable circumstances. I understand the importance of regular attendance in supporting my child's progress and the overall success of the dance program.
2. Tuition and Fees: I will fulfill my financial obligations by paying tuition and fees in a timely manner, according to the established payment schedule. I understand that any outstanding balances may result in my child's inability to participate in classes, rehearsals, or performances.
3. Caregiver Conduct: I will conduct myself in a respectful and professional manner towards the studio staff, teachers, fellow caregivers, and students. I understand that my behavior sets an example for my child and contributes to the positive atmosphere and community spirit of the dance studio.
4. Communication: I will maintain open and honest communication with the studio staff and teachers regarding my child's progress, concerns, or questions related to their dance education. I will promptly notify the studio of any changes in contact information or other relevant details.
5. Studio Policies: I will adhere to all studio policies and procedures, including those related to dress code, behavior expectations, drop-off and pick-up procedures, and studio safety. I understand that these policies are in place to ensure a safe, inclusive, and productive learning environment for all students.
6. Support and Encouragement: I will provide emotional support and encouragement to my child, promoting a positive attitude, perseverance, and a growth mindset. I understand the importance of nurturing my child's passion for dance and celebrating their achievements, both big and small.
7. Studio Events and Performances: I will ensure my child's participation in studio events, performances, and recitals as outlined in the schedule provided. I understand that these opportunities contribute to my child's growth, confidence, and overall dance experience.
8. Confidentiality: I will respect the privacy and confidentiality of other students, caregivers, and studio staff. I will not disclose personal or sensitive information without proper authorization.
Agreement Acknowledgment: I understand that my compliance with the policies and guidelines outlined in the Student and Caregiver Handbook is essential to maintaining a positive and supportive dance community. I acknowledge that failure to abide by these policies may result in disciplinary actions or the termination of my child's enrollment at Honey’s School of Dance.
