Release from Liability
You agree not to hold liable for any acts associated with providing you services at the studio or at remote locations, including MVBT, its owner, staff, representatives, successors and assigns, that result in your sustaining injuries or damages whatsoever. You agree to release and forever discharge the above mentioned from all claims, injuries, damages, risks and actions arising from any of their acts when providing these services.

Assumption of Risks.
You agree to assume the risks of injury and damage from using these services and premises. The risks include muscular, skeletal and cardiovascular injuries. You acknowledge that these and other risks may exist depending on your particular health condition. No representations are made as to the benefits from using these services. We advise you consult a physician as to your particular risks.

Disclosures by You.
Each time you use these services, you agree to disclose to the instructor any adverse health or physical conditions that exist.


Rules and payments.
If you register for any classes, you agree to comply with the rules and payment terms of MVBT. A copy will be given at time of registration.

Miami Valley Ballet Theatre and all staff are pleased to provide you dance, improvisation, theatre, fitness, and other related services. By choosing to use any of these services and in consideration of receiving them at the applicable prices, you expressly agree to the following:

Disclosures by You. Each time you use these services, you agree to disclose to the instructor any adverse health or physical conditions that exist, including: fever, illness, broken bones, sprains, open sores, varicose veins, high blood pressure, pregnancy.
