I, hereby give permission for any and all medical attention to be administered to my child in the event of accident, injury, sickness, etc., at any necessary emergency facility, until such time as I may be contacted. I also assume the responsibility for the payment of any such treatment. This release is effective for the period of one year from the date given below.
If a doctor has ever told you or your child that they should not participate in dance or activity similar to dance, for any reason, a doctor’s note will be required for participation.
I give Center Stage Dance permission to use my child’s photo(s) or video(s) in any advertising, on their website, or any social media. All photos or videos would be those taken in the classroom, at performances, or by our professional photographer Seeley Photo.
*****Performance Policy (Full Season Only)*****
Please note that after the studio spring break if your child misses a lot of classes this could severely put them behind in the recital routine. Multiple absences from the same student can disrupt the learning process of others (think partner work or more complicated formation changes). While we understand and support children being involved in other activities (and certainly don't think we are any more important than another activity) we just ask that you respect the rest of the students in class and try to find a balance where all activities are getting the appropriate attendance. Often times simply alternating weeks with which activity you attend works perfectly.
If your child misses too many classes and is unable to get caught up on a routine, private lessons may be suggested (at the rate of $25/hour). If you do not use the private lesson opportunity to get caught up, please know that the student may need to be choreographed out of the part of the dance they do not know. Other children will not be forced to worry or stress about whether or not someone will show up or be on the right mark or know the partner part, etc. Our goal is for everyone to get on stage and have fun, not stress or have a bad experience because of a classmate. Getting on stage is scary enough for some, so a negative experience could mean the difference in wanting to participate again.
Students are required to be at the last lesson of the season as well as their rehearsal times at the auditorium.
*****Picture/Recital Dress Code Policy (Full Season Only)*****
If your child shows up to pictures and/or recital without the required shoes and/or tights your child will be provided with them and you will be charged the purchase price of these items. Tights are not returnable. Shoes, as long as they are in the same condition, can be returned for a refund within a week. This ensures that the class has a cohesive look for these two events.

All summer session activities must be paid in full at the first lesson.
*****Payment Policy (Full Season)*****
Yearly tuition, costume fee, and recital fee are added together and divided into nine equal payments. These payments are due by the 15th of each month September – May. A late fee of $15 will be applied to all accounts that must be re-billed for a past month’s tuition. This fee will continue to be billed until the account is current. Students with accounts more than 30 days past due will not be allowed to participate.
Tuition is based on a 38-week season that includes 30 weeks of instruction, 5 weeks of breaks, and 3 built in snow/teacher sick days. The snow/teacher sick days are built in simply for convenience purposes. These classes are included whether they are needed or not, and you are not charged for them if we end up not needing them (the ultimate goal is to get you 33 weeks of instruction for the price of 30 weeks!) If a class uses more than the 3 built in days, a make-up class will be offered or a refund will be issued for the missed class.
