I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned, for myself, my heirs, administrators, and executors, hereby waive and release Kristina Weinman individually and NB Dance & Company and it's staff from any and all claims or damages in said program. I further certify that the aforementioned student is in proper physical condition to participate in said program. I, the undersigned, do hereby authorize Kristina Weinman, or her detailed agents (being teachers or administrators employed by NB Dance & Company) to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make NB Dance & Company responsible for payment of medical expenses. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician. This power in in essence a power of attorney and shall remain in effect for one year from the date agreed upon. In addition, I understand and authorize images and videos of my child to be used for marketing and promotional purposes.

I have read and understand that once I register my child(s) I am agreeing to sign up for Auto-Pay and will pay the full amount of my tuition which is due on the First of every month. Tuition will be posted one week in advance, this is when I have the opportunity to pay by cash, check, PayPal or a different card. Autopay will charge all unpaid tuition on the first. I understand that recital and costumes fees will be automatically withdrawn on October and November 15th, unless specified in writing two weeks prior to the due date.

Any decline transactions will result in a $20 charge. A late fee of $20 will be charged on all tuition not paid by the 10th. Please contact peyton@nbdance.net with any questions. If I fail to do so, a late fee of $20 will be applied 10 days after the due date. If I choose to withdraw my student(s) a written or emailed notice must be given two weeks prior to the next due date. If no notice is given, tuition will continue to be charged for the remainder of the season.
Once an account is past due 60 days, your student(s) will be dropped from all classes. I understand that upon withdrawal or dismissal, I will NOT be refunded any paid tuition, recital, or costume fees.

NB Dance & Company is not responsible for any injuries inflicted upon any participating parties. Client(s) will be responsible for themselves and their children and release NB Dance & Company and anyone associated from any claims against their person or their business.
I hereby assume all the risks of participating in any/all activities associated with this studio. including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
HEALTH
I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity, I waive, release and discharge NB Dance & Company and anyone associated from any and all liability. including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage. property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity.
Signature of both parties below indicates the acceptance of this agreement.

I consent that NB Dance & Company may use photographs or videos of my child and me on their marketing and social media platforms, which includes but is not limited to Facebook, Instagram, and Printed Advertisements.
