The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Kelly’s Studio of Dance, Inc. has many preventative measures in place to reduce the spread of COVID-19; however, Kelly’s Studio of Dance, Inc. cannot guarantee that you or your child(ren) will not become infected with COVID-19. Attending any public gathering could increase your risk and your child(ren)’s risk of contracting COVID-19.


By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Kelly’s Studio of Dance and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Kelly’s Studio of Dance may result from the actions, omissions, or negligence of myself and others.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Kelly’s Studio of Dance or participation in programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Kelly’s Studio of Dance, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Kelly’s Studio of Dance, Inc., its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Kelly’s Studio of Dance, Inc., class(es) or programs.


BY REGISTERING AND PAYING FOR ANY CLASS, EVENT, PRIVATE LESSON, OR OTHER ACTIVITY THROUGH THIS WEBSITE, YOU ARE AGREEING TO ALL OF THE ABOVE CONDITIONS AND WAIVERS. IF YOU ARE AN ATTENDEE OF OUR REGULAR CLASSES AND HAVE REGISTERED AND ENROLLED IN A CLASS THROUGH THE “PARENT PORTAL” YOU AGREED THROUGH THERE AS WELL. PLEASE DO NOT HAVE YOUR CHILD/YOURSELF PARTICIPATE IN ANY CLASS OR EVENT UNLESS YOU ACCEPT AND AGREE TO ALL OF THE TERMS IN THIS AGREEMENT.

We will take special care to protect the privacy and security of your information. This policy explains our information practices and the way that Kelly's Studio of Dance, Inc. collects and uses your information. This Privacy Policy covers the information you provide to us online and on location at Kelly's Studio of Dance, Inc.

When you register for classes, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, e-mail address and student information such as student names, birthdays, medical conditions, and your comments. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below.

When you register, you are required to enter an e-mail address and a password. The combination of that e-mail account and password is used to ensure that only you have access to, and only you have the ability to modify, your personal information through Kelly's Studio of Dance, Inc.'s website. Kelly's Studio of Dance, Inc.'s office personnel has access to your personal information only to manage the studio business.
