Compass Dance Academy Waiver and Release Liability 2023-2024
I have chosen to have my child to participate in dance instruction given by Compass Dance Academy, LLC. Dance instruction that includes and not limited to: ballet, pointe, baton, jazz, tap, lyrical, contemporary, introduction to dance, adaptive dance, hip-hop, cheer, acro-cheer, acrobatics, musical theatre, conditioning classes and dancer yoga. I acknowledge, I understand the nature of the activities my child will be participating in and the possibility that despite precautions, accidents and/or physical injury may occur. 1/we understand it is our responsibility to inform Compass Dance Academy, LLC. teachers and staff of any and all medical and physical conditions. Our child, is released by a physician to participate in any & all dance instruction. If a change presents during this year it is our responsibility to inform and update Compass Dance Academy, LLC. Instructors and staff members.
1/we agree to release and hold harmless Compass Dance Academy, LLC including its teachers, and staff members from any cause of action, claims, or demands now and in the future. 1/we will not hold Compass Dance Academy liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during or after classes or which may occur at any Compass Dance Academy LLC sponsored event outside the studio. Events that include and not limited to: on-site dance instruction, parade participation, competition participation, fundraising events, performance opportunities, conventions & other educational learning opportunities, off site rehearsal and recital participation.
Furthermore, I/we agree to read the Compass Dance Academy policy handbook and take full responsibility for my/our behavior in addition to any damage 1/we may cause to Compass Dance facilities and other facilities used by Compass Dance Academy, LLC. I authorize and agree that Compass Dance Academy, LLC. may take and use photographs, videos of my child as needed for record keeping, advertising, social media and/or public relations projects and that I have no rights to the same and will not be compensated for the same by Compass Dance Academy, LLC.
My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.
I have fully informed myself to the contents of this release and have read the same prior to agreeing to this waiver and liability release form. I agree to this release by registering my child at Compass Dance Academy.
