You must be over 18 to register for an account. You agree to all terms and conditions of MainStage School of Performing Arts.

You release MainStage School of Performing Arts and all its employees from all claims on account of any injury which may be sustained by your child while attending any class or event associated with MainStage School of Performing Arts or outside performances. You also acknowledge your responsibility in paying monthly tuition, for any associated costumes, players fees, and all other communicated costs involved. You agree to give MainStage School of Performing Arts and Children's Theatre and any sponsoring agency permission to use any photos or videos of your child to promote MainStage. You give your permission for your child to be treated at MainStage and any outside venue sponsoring MainStage, for any medical non-emergengies. This is restricted to Band-aids, Neosporen to generic brand of antibacterial cream, ice-packs, compresses and Tylenol or Ibuprophan (Teens only).

I support MainStage School's Covid 19 policy of masks and social distancing. I understand that despite safety precautions, protection against the virus is not foolproof. I will not hold MainStage School of Performing Arts or MainStage WestEnd Theatre liable of virus transmission and for any costs incurred should my child contract the virus. I will inform MainStage immediately if my child or anyone in my family contracts the virus and will voluntarily refrain from attending MainStage or any of the WestEnd productions until it is proven that the child and/or family member is no longer contagious. I will notify MainStage immediately if my child or family member is exposed to the virus and will not attend any classes or events for a period of at least 10 days from the time of exposure. I understand that MainStage will comply with any orders from the state of Kentucky or nationally concerning closures. I will furnish any information needed in case contact tracing is warranted. I understand that MainStage cannot prorate and refund fees dues to closures or quarenteens.

I agree to pay MainStage School of Performing Arts all fees including registration, tuition, show fees, costume charges and any other fees charges by MainStage in a timely manner. I understand that a late fee will be charge for past due balances. I also understand that should my child is no longer enrolled at MainStage, I am still responsible for any fees charges up to the date of dropped classes. I understand that I will be charged a 10% late fee for all past due accounts. And account is considered past due, 10 days from the due date.

I agree and give permission MainStage School of Performing Arts and MainStage WestEnd Theatre to use photos and or videos of my child(children) for promotional purposes. This includes social media, the website, and hard copy (flyers, program, brochures,etc.).
