I understand that by signing up for classes at EDC I am agreeing to read and abide by the policies set forth in the Family Handbook found on my portal.

[Release of Liability]
I understand that I have chosen to have the above mentioned student participate in dance instruction given by Evolution Dance Co., LLC. I acknowledge that I understand the nature of the activities my child will be participating in and the possibility that despite precautions, accidents and/or physical injury may occur.

I/we agree to release and hold harmless Evolution Dance Co., LLC including its teachers and staff from any cause of action, claims, or demands now and in the future. I/we will not hold Evolution Dance Co., LLC liable for any personal injury or any personal property damage or loss, which may occur on the premises before, during or after classes or which may occur at any Evolution Dance Co. sponsored event.

I agree to allow Evolution Dance Co., LLC to post photos of my child to social media and their website. I will alert EDC in writing if I do not want photos of my child to be posted.

[COVID-19/Health Waiver]
Evolution Dance Co., LLC will be following all Washington state regulations and will be transparent in it's protocols and efforts to maintain a safe and clean environment for dancers of all ages to move in. Upon reopening our physical studios, we will let customers know what kind of protocols will be instilled. Due to these unprecedented times, no business entity or individual can 100% guarantee there will be no risk in transmission of COVID-19 in their place of business. As such, Evolution Dance Co., LLC cannot guarantee that there is no risk of spreading of COVID-19 or any other illness in our studio locations or any other location at which we may hold classes or events. By signing this waiver you agree to release and hold harmless Evolution Dance Co., LLC and its employees for any risk and transmission of COVID-19 or any other infectious disease that may affect you, your child or anyone you come in contact with. We will be doing our absolute best to protect our customers during this time.

I give permission to Evolution Dance Co., its owners and instructors to seek medical treatment for my student in the event they are not able to reach a parent or guardian.

[Payment Information]
*Late fees will apply if tuition is not paid on time. Tuition is due the last day of the month prior. $2 late fee will be applied every day past the due date.*
