(1) Medical Waiver - Certify that all registered dancers are healthy enough to participate in physical activity and dance. Release ONE Academy of Dance from all liability and agreement to hold harmless ONE Academy of Dance.

(2) Tuition Agreement - Understand that tuition is due in full on the 1st of the month. A late fee of $15 will be charged on the 9th. A fee of $50 will be charged for accounts that are 15+ days past due. First and last month’s tuition is due at the time of enrollment and are non-refundable. Payment for the year of tuition is divided into monthly payments for the ease of our families. June is a half tuition payment and is due with the tuition for the month you enroll.

(3) Drop Policy - ONE Academy programs are designed for the full school year. I understand and agree that a minimum 30 days written notice is required to drop a class. Families must send an email no less than 30 days from when they wish to unenroll. There are NO exceptions to this as it can affect both the teachers who have committed to teach the class and the other students who have committed to attending the class.
