RELEASE FROM LIABILITY
I have chosen to have my child participate in dance instruction given by Claim to Fame LLC. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Claim to Fame LLC is not in any way responsible for making such a determination. In consideration of my child’s enrollment in any dance instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Claim to Fame LLC from all claims, costs, liabilities, expenses or judgments, including attorneys’ fees and court costs for any occurrences in connection with any dance instruction. I assume all risks to my child in connection with any instruction and further release Claim to Fame LLC and its owners and employees from liability for any injury sustained by my child while he or she is enrolled in any
dance instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen. I understand that Claim to Fame studios is not responsible for personal property that is lost, damaged or stolen while I or my child is at or on Claim to Fame property. I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in Claim to Fame LLC activities and that Claim to Fame LLC does not provide accident or health insurance for those participating in its instruction, activities or programs. My clicking and accepting this document via Dance Studio Pro is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to agree to this affirmation.
AGREEMENT OF CLASSROOM PROTOCOL:
I understand that the accepted practice and methodology of professional dance
training entails “hands on†and other forms of physical contact with instructors and other students, and that this is necessary for a student to learn at an appropriate pace. I understand that if there is currently, or ever would be, any reason why I should not be touched, or should only be touched in a specific way, that it is my responsibility to inform each instructor as to what is acceptable, and to discuss any related physical or mental issues with them.
STUDIO POLICIES
By registering for any class, session or program at Claim to Fame LLC, all students and parents/guardians indicate their awareness, acceptance and consent of all studio Policies, Procedures and Waivers as detailed on this website, which may be supplemented or amended from time to time. In that event, notification will be sent to all active accounts by email.
UPDATED SAFETY MEASURES & PROCEDURES
The health and safety of Claim tonFame’s students, faculty and families is our primary concern and constant focus. We have implemented a sanitizing daily cleaning of the entire building, i.e., all studios, lobby, and common areas.
PAYMENT
1. TUITION: The annual tuition for dance lessons is based on 30 regular classes for the period of September- June. Monthly fee equals 1/10 of the annual fee and is not based on number of weeks/classes in a month. All holidays and vacations have been accounted for. Tuition is due by the 1st of each month. We accept credit card and personal checks for payment. If you decide to pay with a credit card, we will place you on Auto Pay which will charge your card automatically on the 1st of each month. If you're utilizing the online registration, payment will be processed immediately. If you do not wish to be placed on Auto Pay, you must contact the Studio Manager by the 1st of each month. All families with students enrolled in on-going classes MUST have a valid credit card on file with the front desk regardless of their decision to use Auto Pay. Autopay can only be canceled by contacting our front office
2. LATE FEE:Â Tuition that is not paid by the 10th of each month, is considered LATE. You may pay for classes early if you know that you will miss the first week of class. If tuition is not paid by the 10th of any given month, a late fee of $10 will be assessed. If tuition is not paid by the 20th of any given month, a late fee of $25 will be assessed and the student will not be allowed to participate in any further classes until tuition and late fees are paid in full. After the 25th, if you fail to communicate a payment plan and ignore all statements sent, your credit card on file will be charged. No exceptions. If payment is not made by the 1st of the following month, classes will be suspended for your dancer.
3. REFUND POLICY:Â There are NO REFUNDS OR CREDITS that will be given for tuition, registration fee, recital tickets, recital costumes, and any tuition related fees. There are no refunds for missed classes or inclement weather closings.
4. REGISTRATION FEE: Our annual registration fee of $20/student is assessed every March. If this fee is not paid by the tuition due date, then your child will be dropped from their class. This is a non-refundable fee. Please note that if your card is on auto-pay with us, this fee will automatically be taken out on the 1st with your child's tuition.
5. PRO-RATING:Â Â We do not prorate classes for existing students. NO EXCEPTIONS.
6. RETURNED CHECKS: Any check that is returned will be subject to $34 fee.
7. CANCELLATION: I understand and agree that Claim to Fame LLC reserves the right to cancel any or all classes. Should a class be cancelled, we will provide an acceptable replacement.
I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ THE SAME PRIOR TO CONFIRMING AGREEMENT.
