Waiver of Liability: I agree that I will not hold Portland Ballet or Portland School of Ballet (PSB), any of its faculty, administration, employees, or guest teachers liable for injuries or illness contracted by me / my child while a student of PSB or as a consequence of participation in the PSB program/classes, or events. I have read, understand, and agree to all of Portland School of Ballet's Policies. I also agree that Portland Ballet has permission to use my / my child's contact information for class purposes only and will never be given out to a third party without consent. Class purposes include, but are not limited to: notification of class changes, arranging of rehearsal schedules, cancellations, etc.
Likeness, Videos, and Photography: I agree that Portland Ballet and Portland School of Ballet have permission to use photographs and video footage of me / my child, for advertising purposes that include print, online, TV, and others. To opt out of the photo waiver, please contact office@portlandballet.org
Responsible Party: I agree that I am responsible for payments for the student I am registering. I also agree that Portland Ballet / Portland School of Ballet may contact me in regard to any payment issues regarding the student.
