Payments are responsibility of parent or guardian of the minor student and must be paid on time. Failure to do so will jeopardize participation in annual recital. Brodhead Dance Academy, its affiliates, owners, and employees are not responsible for accidents, thefts, or injuries and shall be held harmless.

PHOTO USE RELEASE FORM

I hereby grant and authorize Brodhead Dance Academy the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures or videos taken of my dancer for promotional materials, including but not limited to newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, submissions to journalists, websites, social media, and other print or digital communications, without payment or other consideration.
This authorization extends to all languages, media, markets, and formats now known or hereafter devised. This authorization shall remain in effect indefinitely, unless I revoke it in writing.
I understand and agree that these materials will become the property of Brodhead Dance Academy and will not be returned.
I hereby release and hold harmless Brodhead Dance Academy from any and all liabilities, claims, demands, or causes of action, whether known or unknown, arising out of or related to the use of these materials. This release is binding upon me, my heirs, representatives, executors, administrators, and any other persons acting on my behalf or on behalf of my estate.
