We agree to read over the entire membership and will follow the rules for the Champagne Academy of Irish Dance. This Agreement also covers our yoga classes.

Legal Release and Policy Acceptance
MEMBERSHIP AND RELEASE OF LIABILITY Please return this completed page, along with your payment directly to Ms. Champagne. PLEASE UNDERSTAND THAT YOU MUST AGREE TO ALL OF THE CONDITIONS IN OUR MEMBERSHIP TO BE A MEMBER AND BE IN OUR ACTIVITIES.
1.In consideration of (PRINT dancer’s name and class):___________________________________________________ (Dancer) (“Participantâ€) being allowed to participate in dance training, activities, and programs of Champagne Academy of Irish Dance, the undersigned do hereby forever waive, release and discharge T. Champagne and the Champagne Academy of Irish Dance, and its officers, agents, employees, representatives and executors, as well as all others acting on behalf of T. Champagne and/or the Champagne Academy of Irish Dance, LLC (collectively referred to hereinafter as “CAIDâ€), from any and all claims or liabilities for injuries or damages to Participant and/or Participant’s property, including those caused by the negligent act or omission of any of the aforementioned parties, arising out of or in any way connected with Participant’s participation in any activities, programs or services of CAID. The undersigned agree that the release of liability provided for herein is intended to be as broad and inclusive as permitted by the laws of the States of Iowa and Illinois, and if any portion of this Release is held invalid, it is agreed that the balance shall continue in full force and effect. The undersigned are aware that dance can be a taxing and strenuous activity and that the Participant could suffer minor or serious injuries, including death, while engaging in activities, programs or services of CAID. Participant is voluntarily participating in these activities with knowledge of the danger involved and agrees to assume any and all risks of bodily injury, death or property damage, whether those risks are known or unknown. I consent to my child receiving medical attention and treatment if necessary. The undersigned also agree that this Release shall be binding on Participant’s spouse, heirs, successors in interest, parents, guardian(s) and legal representative(s), in addition to Participant, and no claim shall be made in connection with Participant’s participation in any activities, programs or services of CAID by any of said individuals.The undersigned have carefully read AND agree to this Release and the Membership Policies and Procedures, and fully understand and agree to all of its contents including, but not limited to: dancer and family expectations (including class, feis, and performances), policies, choreography and copy-writes, and fees. I acknowledge that I have received and reviewed a copy of the policies and procedures of Champagne Academy of Irish Dance, and agree to be subject to, and comply with, all of said policies and procedures. I further acknowledge that I understand that the policies and procedures of Champagne Academy of Irish Dance, are subject to change and I agree to review and comply with any new policies and procedures of Champagne Academy of Irish Dance, of which I receive notice. The undersigned are aware that this is a release of liability and a contract between the undersigned and CAID and the undersigned sign this Release of their own free will. The undersigned understands that all CAID choreography and any affiliated steps and costuming is copy-written and school specific, and may not be used in any way outside of official CAID events – in practice or performance without written permission form Trea Champagne TCRG. Proper notification is to be given for requests. The undersigned understands that Champagne Academy of Irish Dance and Gra na Gaelige Iowa Celtic Arts is a performance based art and may use photos and videos of my child, or me if I am an adult student, for marketing purposes including, but not limited to, flyers, posting on Facebook and other social media, as well as posting on the webpage of Champagne Academy of Irish Dance and Gra na Gaelige Iowa Celtic Arts. I understand that it is my responsibility to ensure that the security settings on any social media of which my child, or me if I am adult, is a member are properly adjusted in order to protect my privacy, to the extent, I have privacy concerns. I understand that photography or recording lessons or classes is not permitted without permission from Trea Champagne. In the event Participant is under the age of 18, and only one parent or legal guardian signs this Release, the parent or legal guardian so signing agrees to indemnify and hold CAID harmless against any claims of the other parent or legal guardian made in connection with Participant’s participation in any activities, programs or services of CAID. Respect and discipline are essential to instruction. All students and families must show courtesy and be respectful to those involved with CAID and as well as those who are not with CAID. All students and families are expected to be respectful to all teachers, dancers, participants, and family members in all classes, performances, feis, and at events, in person and online. .Please arrive 5 minutes prior to your class to change and be ready to participate. Children cannot wait in the lobby by themselves; an adult must supervise them. Wait in the designated waiting area (and not in the classroom) until your class starts. Dancers are to leave side conversations out of the studio and enter prepared to focus, dancers should not talk when a teacher is talking or teaching. Gossip is not allowed. Please review the dance etiquette file in our Dance Studio Pro account if you are unfamiliar. All persons are responsible for their belongings, their trash, and must pick up after themselves at any venue we may use. In the unfortunate event that a dancer is injured and unable to dance until healed, dancers should continue to attend their regular classes, so that they can continue their instruction through observation and discussions within their class. No refunds are given. No dancer or parent is allowed to speak for CAID without the explicit acknowledgement and approval in writing. In the event any CAID member is not courteous and respectful, in person or online, in class or at an event, or does not follow CAID guidelines, we will ask the student/guardian to correct the problem. If the situation is not resolved, we may ask the student to watch quietly until they are ready conduct themselves properly and return to the lesson. In the event the member does not comply, the member will be asked to discontinue their class. No refunds are given. The undersigned understands that our students may not belong or participate, on their own or with another Irish dance class, group, workshop, or organization that performs, teaches, or offers any Irish dance instruction. Then undersigned understands that tuition and costs are based around our enrollment and therefore, NO REFUNDS of any kind are given for merchandise, tuition, or registration. In the event that our school is not a good fit for your family at this time, please email and talk to Ms. Champagne before the end of your term to help with the transition.
