re.DEFINE Waiver and Media Release Agreement
- I understand the physical activity that I/he/she will endure as a participant of reDEFINE Dance Studio that includes but are not limited to dance, auditions, rehearsals, classes, workshops, conventions, performances, and/or competitions. I am fully aware of the inherent risks involved with dance and group activities, and I volunteer myself/him/her to participate in reDEFINE dance company and affiliated events.

- I assume full responsibility for any risks of loss, damage, injury, paralysis, or even death to myself or my child during the supervision of re.DEFINE. I further agree to hold harmless re.DEFINE Dance Company and affiliates, Soulidified Project and affiliates, as well as Impact Martial Arts directors and affiliates, for any/all loss, damage, injury that I/he/she may incur during reDEFINE events/activities. I know of no physical reason that would prevent myself/him/her from participating in any reDEFINE events/activities.

- I understand that individuals should be covered by insurance policies held by myself or parents/guardians.

- I understand that I/he/she may be photographed or videotaped during reDEFINE events/activities. I hereby release any footage of myself/him/her to reDEFINE dance company and its directors, for any/all promotional and informational uses.

- I hereby release, waive, discharge, agree not to sue, and agree to hold harmless for any and all purposes reDEFINE Dance Company, Soulidified Project, Impact Martial Arts and all affiliates, from any and all liabilities, claims, demands, or injury, including death, that may be sustained by myself/him/her while participating in any reDEFINE events or activities, or while on the premises of reDEFINE Dance Studio or any other dance venue.

I certify that I have read the above waiver in its entirety, and I agree to all terms stated above. I certify that all information I provided is true. I give full permission for the participant to be in all reDEFINE events/activities, unless I otherwise i*

I understand the physical activity that I/he/she/they will endure as a participant of reDEFINE Dance Studio that includes but are not limited to dance, auditions, rehearsals, classes, workshops, conventions, performances, and/or competitions. I am fully aware of the inherent risks involved with dance and group activities, and I volunteer myself/him/her/them to participate in reDEFINE Dance Company and affiliated events.

re.DEFINE Dance Studio cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while utilizing the studio, attending outdoor rehearsals and classes, and all re.DEFINE Dance Studio Events. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize service and/or enter onto our studio you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against re.DEFINE Dance Studio, Tehani Dance and Fitness, Impact Martial Arts and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing re.DEFINE Dance Studio services and studio. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE. BY ATTENDING, I FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE
