As the legal parent or guardian of a dancer(s) enrolled in classes at Vicki's School of Dance, I release and hold harmless Vicki's School of Dance, its owners, operators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, illness or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Vicki's School of Dance, its owners, operators and instructors or in route to or from any of said premises. The undersigned gives permission to Vicki's School of Dance, its owners, operators and instructors to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. In consideration of being allowed to participate on behalf of Vicki's School of Dance and related events and activities, the undersigned acknowledges, appreciates, and agrees that participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Vicki's School of Dance, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“releasees”), with respect to any and all illness, disability, death, or loss or damage to person or property, whether arising from the negligence of releasees or otherwise, to the fullest extent permitted by law.
