The authorized legal representative jointly hereby forever releases, discharges and acquits Hit the Floor Studio of Dance Ltd and all their instructors, employees, investors, agents, owners, and directors from any and all claims for damages or injuries of any kind, nature or description. This waiver is to be effective on completion of registration. In the absence of a signature, by paying the registration fee you hereby agree to the terms and conditions of the registration form.
I agree to pay all dance fees on time.
I understand that all fees paid to Hit the Floor studio of Dance Ltd are non-refundable. There will be no refunds issued for withdrawal out of our programs. If you withdraw before the season begins, a credit will be applied to your account for further use.
I understand that there may be additional costs associated with my child’s Dance class. Proper Footwear, Tights, Photos, Competition fees, Motel/Hotel fees, and Travel, are the sole responsibility of the Parents or Guardians.
Parents or Legal guardians give their permission to Hit the Floor Studio of Dance and their affiliates to use photos and or video of their child.
By registering, all parents and/or guardians agree to hold harmless from any and all liability Hit the Floor Studio of Dance Ltd,its officers, employees and contractors both in their professional capacity and personally for all injury or illness resulting from or in any way connected with his/her participation in the classes, activities or special events at the studio. Hit the Floor Studio of Dance Ltd assumes no responsibility for lost, misplaced, or stolen articles. Please put your name in all your belongings and do not leave valuables unattended.
I grant permission to the staff of Hit the Floor Studio of Dance Ltd to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio. In case of medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the emergency unit deems it necessary. I understand that in some medical situations the staff will need to contact the emergency resource before the child’s parent, physician, and or other person acting on the parent’s behalf. I also understand and agree that the child’s parents or legal guardians shall be responsible for any expenses incurred.
