I hereby represent and warrant that if the participant is a minor, I am his/her guardian and authorized to provide the release, authorizations, and permissions as stated below and all information above is accurate and complete. I hereby give permission for the applicant to participate in all dance program activities and agree to release DAmour School of Dance, officers, employees, facilities and agents, from liability arising from any harm or injury incurred by the participation of my child in the dance program including online. Unless otherwise indicated by a parent/guardian in writing at this time of registration, photographs & videotapes of participants for use in D’Amour School of Dance publications may be taken while participating in the dance program activities. No personal information other than the participants name will be released under any circumstances except as required by law. I am also aware that I hold responsibility to any harm that this minor or I have done to the props & equipment and instruments used in class as it is my responsibility to cover any damages done to DSOD property while in my care. Costs can range from $50-$600 per item. By signing below, I also agree that my child does not convey any Covid-19 symptoms and release DAmour School of Dance, LLC, it's owner and representatives from any liability for the unintentional exposure or harm due to COVID-19.

A Registration Form that acts as our waiver must be signed upon attending a class/event. This form must be completed in order for your participant to dance with DSOD for any class/event.
A $50 Membership Fee is mandatory for all students for each full dance year.
Tuition only covers the technique class enrollment. Additional fees may apply for extra items/classes/performances. All fees are applied to your account with monthly payment installment options. Please reach out to us if you have any questions.

Once enrolled in our technique classes per season, an updated credit card is required on file for Auto Pay to complete the dancer's tuition.
All monthly installment plans are processed on the first of the month with a 5 day Grace Period. On the 6th day of the month is your payment is not completed, a $25 Late Fee will be applied to your account with no exceptions.

We do accept Venmo, Cash App, Paypal, and Zelle payments. Payments on outside sources must be completed the day before the due date so your credit card payment will be cancelled. A receipt will be emailed once the payment is recorded in DSP.
