You must be over 18 to register for an account.
It is customers responsibility to check that total is correct before completing purchase.
-Anyone taking classes at Switch Performing Arts may not participate in another studio's performance company, team, or any other group that represents another dance organization.
-Any student of Switch Performing Arts may not instruct or be employed in any capacity at any other facility teaching dance.
-Any student of Switch Performing Arts may not volunteer as an instructor or performer without prior approval from the Studio director.
*Breaking any of the policies listed above will result in immediate cancellation of your enrollment without refund.*
-Students will not engage in any illegal activity while on the studio premises.
-Students will remember that Switch Performing Arts is a family environment and will be appropriate in their dress, speech, and behavior while on the premises.
-Students will remember that we are a dance family and be friendly and respectful of each other at all times.
(A) I am aware that performing arts training and equipment carries risk of physical injury. On behalf of my child, ward, and myself, I expressly assume that risk and agree that Leo Gallegos, Switch Performing Arts (SPA), and its staff shall not be liable in any way for any personal injuries caused by, or arisen from, the above described activities or any activities related thereunto during attendance at SPA, included but not limited to misuse of equipment and/or the student's physical conditions and limitations. (B) Further, I expressly grant SPA and its employee's permission to summon qualified EMT personnel and hereby authorize any emergency treatment that may be required for my child, ward, or myself. (C) I expressly authorize SPA staff to provide first aid to my child or ward for minor cuts, scrapes, sprains, etc. I stipulate that I have medical coverage. If my child, ward, or myself has allergies or special needs, I will list them in the notes of the student profile. (D) I hereby give SPA unrestricted right and permission to take, use, and publish photographic and/or video images of my child, ward, or myself, without compensation, for promotion, advertising, or any other legitimate and legal purpose.
I understand that tuition at Switch Performing Arts is due on the first day of every month. I understand that it is late after the 7th of every month and will incur a $10 late fee per student. I agree to keep a valid credit card on file with Switch Performing Arts that may be used to execute the following policy:
-Switch will notify you on the first of the month of your balance and that tuition is due.
-Switch will give a second notification if you have not paid by the 7th that your tuition is late and has incurred a late fee on the 8th.
-Switch will charge the card on file for the outstanding balance on your account on the 11th of the month.
-Switch will never charge the card on file automatically for tuition unless you have specifically opted in to automatic payments on the first. Tuition may be stopped at any time with notification 2 days prior to the first of the new month.
-There is no pro rating of tuition for skipped days. Missed lessons may be made up by attending another class of the missed age group.
-If a student misses an entire month and does not pay tuition for that month, a $25 reinstatement fee will be charged and paid before the student may return.
-attendance policy for recital is as follows:
-any class absence within 30 days requires a make up private at the studio's current private lesson rate.
(This is per absence and not per day.)
-No absences are allowed within 1 week of recital for any reason.
-If you miss your dress rehearsal time, you will not be allowed to perform in that classes dance.
*This policy is in place to protect the hard work of all the dancers as well as the teachers. No exceptions will be made to the policy to ensure fairness for all.*
-All outstanding balances must be paid in order to perform in the recital.
