WAIVERS

Prior to Registration please carefully read and you will be asked to agree and acknowledged the Pro Dance Elite standards, waivers, and policies.

Release of Liability
In consideration for (dancer's name) ________________________________'s participation in the activities, classes, rehearsals, events, performances, provided by Pro Dance Elite, Inc., including but not limited to all aspects of dancing, conditioning, and training and/or competition. I am fully aware that any activity involving motion, height, or athletic activity creates the possibility of serious injury. I hereby release Pro Dance Elite, Inc., including its officers, shareholders, agents, contractors, and employees, from any liability to the above named dancer, of the person claiming through him/her, arising from injury to the person or property of the above named athlete occurring on the premises of Pro Dance Elite Inc., including any event sponsored or sanctioned by Pro Dance Elite, Inc., and/or travel to and from such activities. This release included but is not limited to any claims of negligence, dangerous condition, latent defect,. Premises liability, code violation, negligent security, failure to warm, vicarious liability, negligent hiring, negligent supervision, negligent maintenance, or improper/dangerous equipment; it is intended to be as broad as permissible under Illinois Law. I am fully aware of the nature of the activities provided and the possibility of injuries arising from such activities. I further agree to hold harmless, indemnify and defend Pro Dance Elite, Inc., including its officers, shareholders, agents, and employees from any loss, liability, damage, or cost incurred by them due to the above named dancer on the premises or successor in interest, and anyone claiming by or through him/her. In addition, I have read and understood the registration form and agree to all terms as stated above. I also attest that all given is factual. I certify that the athlete is in good health and may participate in activities at Pro Dance Elite, Inc. In case of an emergency requiring medical treatment, the undersigned hereby authorizes Pro Dance Elite, Inc. and its agents, to take the above named dancer to a qualified medical or hospital facility for care and treatment.

As the legal parent or guardian, I release and hold harmless Pro Dance Elite, Inc., its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Pro Dance Elite, Inc., its owners and operators or in route to or from any of said premises.

Medical Emergency
The undersigned gives permission to Pro Dance Elite, Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician ________________ be called and that my child be transported to ______________________ hospital. Please include physicians' phone number _______________.

Photography and Video Release
I grant to Pro Dance Elite, it's organization, instructors, and/or promotional companies, the right to take and use photographs and videos of said dancer(s). I authorize Pro Dance Elite and the before mentioned organizations, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Pro Dance Elite, Inc. may use such photographs and videos of me and/or my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

Payment Information
Tuition is due by the first of each month. If accounts are paid after the tenth of the month, there will be a $25.00 late fee applied to the account balance. There is a $45.00 returned check charge for any checks returned by the bank. A 30 days written notice is required to drop any classes this excludes company. To drop a class, 30 Day written notification is required to pdeoffice@yahoo.com stating the student name, class name and reason for drop.There are no exceptions to this policy (applies to company dancers for non-company classes). Please do not call or stop into the office to notify us of a drop.

Your financial obligation for the class is not complete until the end of the 30 day period. You are responsible for tuition until that time and are welcome to continue to attend class(es) until that date. Drop fees will be charged at the time of the drop notice receipt. This policy does not apply to Company classes which can not be dropped until the end of the season. Please refer to your Studio Handbook and registration form for company policies and requirements.

I understand that by registering for classes I automatically acknowledged that I have read and agree to all the above terms of the five liability waivers.

I agree that Pro Dance Elite, Inc. may use such photographs of my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. I understand the risk of Covid-19 exposure and hold harmless Pro Dance Elite it's employees, instructors, & officers.

Financial Agreement

A 30-Day Written Notice must be given to drop any class. Tuition is DUE for that following 30 days and will be charged at the time of the drop notice. Dancer may take class during their 30-day notice period. No exceptions are made on this policy.

Tuition is DUE on the 1st and Autopay payment processed on the 1st of each month. All accounts must have a card on file for Autopay. By putting your card on file you agree to your card being charged monthly for tuition, late fees, or other account fees. I understand that if I wish to pay other than using Autopay I need to have payments in FULL PRIOR to the 1st of the month.
