I hereby grant permission for photographs and video recordings to be taken of me during classroom instruction, performances, competitions, and other STARZ related activities. I understand that this media may be produced and shared for educational and promotional purposes. I authorize STARZ Performing Arts Academy to use my photos or videos on their website, Facebook page, Instagram, Twitter, Newspapers, promotional literature, etc. without further consideration, and I acknowledge the school’s right to crop or treat the media at its discretion. I also acknowledge that STARZ may choose not to use my photo at this time, but may choose to do so at a later date. I am aware that STARZ reserves the right to discontinue use of photos without notice.

I understand that dance, gymnastics, and aerial arts can be a vigorous physical undertaking and that I have the right to withdraw from any activity I consider beyond my child’s capabilities. I, the undersigned, do hereby voluntarily submit my application for my child’s attendance and participation in STARZ Performing Arts Academy’s programs. I release STARZ and its promoters and instructors from any claims or liabilities for any injuries, damages, or losses that may occur while in participation in the dance, aerial arts, and/or gymnastics programs. My child and I have carefully read and understand STARZ Policies and Guidelines and plan to uphold studio rules and dress codes, so that my child may get the most out of their performing arts education.
