I, the (parent/legal guardian) acknowledge that I AM or MY CHILD is signing up for instructions in a performance class that may involve physical exertion and personal body contact. As a condition of being admitted to the program offered by First Dance, I assume all risk of injury and hold the instructors and company harmless from any and all liability (including attorney's fees) for all claims, action, or damage arising from any injuries suffered to your child arising out of activities involving dance or physical activity, whether occurring on the premises or elsewhere, excepting only those claims, actions or damages caused by gross negligence or intentional act or omission of First dance or their duly designated representatives. By enrolling in this program and paying of fee, the parent or legal guardian attests that the student has had a proper medical check-up and is fit to undertake physical activity.

Photography - I hereby permit First Dance to use, in whole or in part, photographs and or videos of the above named student for the purposes of illustration and publication including our website.
