PLEASE NOTE: All tuition payments are due by the 1st week of the month before the student begins a class for that month. We accept e-transfers, cash and cheques for payment. Payments are nonrefundable once classes begin. A $20 fee is assessed on all returned checks *Credit card payment is accepted. Rates for credit card payments require 3% of the total amount to be added to the final total. Thank you for your cooperation with our policies. I understand and am familiar with the classes and programs offered at CSOPA that I choose to participate or enroll in. I acknowledge that there is a risk of injury when receiving instruction and at the times when I am without instruction. In consideration of the facility and location provided to me/my child, I forever release, discharge, and hold harmless CSOPA and any of its employees, representatives, or members from all liability or claim. Medical Emergency: I consent for any employee or representative of CSOPA to seek reasonable medical treatment, emergency or otherwise, at any time for me/my child. I agree to be liable for the cost of such treatment. Additional Waiver: I hereby grant CSOPA permission to lawfully photograph and/or videotape my child for educational or marketing purposes.
REFUNDS and WITHDRAWALS: The registration fee is non-refundable. If your child withdraws from class within 48 hours of the second class you will be required to pay the full first month. No refunds are given after the second week of any year-long performance class unless requested for medical reasons. If your child withdraws from classes after that time full fees for the program will be due and payable. ATTENDANCE and PUNCTUALITY: I acknowledge that I/my child is expected to be on time for rehearsals to fully participate and not disrupt the other students. My child is expected to attend all rehearsals, including extra rehearsals, up to and including show time or last class. I will advise the director/instructor as soon as possible before a rehearsal. I understand that prolonged absenteeism or behaviour issues may result in casting an understudy or recasting. PREPAREDNESS: I understand that I/my child needs to be prepared for each class. Being prepared means showing up in proper attire, including dance footwear; with your script, pencil and water bottle; and practicing at home each day. Parents are expected to read emails and written notices to be aware of any changes so their child is in attendance when required. PERSONAL/MEDICAL INFORMATION: Information provided on all forms is considered your property and will not be shared with any third party. It helps us greatly to know of any learning challenges or struggles that your child may be dealing with. Success looks different for every child. We have considerable knowledge in aiding in the success of each child. It can be as simple as letting us know that their pet is unwell or as complicated as diagnosed or undiagnosed medical issues. We will keep all information private and confidential.
