EK Dance Academy – Waiver

As the legal guardian of my designated student(s) (student(s)), I hereby consent to all student(s) participating in the EK Dance Academy program(s). I recognize that potentially severe injuries can occur in any activity involving motion or height, including dance and related activities including jazz, tap, ballet, pointe, hip hop, contemporary, lyrical, acrobatics, pyramids, yoga, pilates and physical activity in general. I understand that it is the express intent of all staff and personnel to provide for the safety and protection of my student(s) and, in consideration for allowing my student(s) to use these facilities, I hereby COVENANT NOT TO SUE and FOREVER RELEASE this facility, affiliated and partner companies and organizations, property owners and lessors, staff, contractors, subcontractors, teachers, coaches, owners, directors and other members involved in this facility's program(s), from all liability and for any and all damages and injuries suffered by my student(s) during instruction, supervision, and/or control during any and all classes or extra activities.

Visual Release
I grant permission for my child and/or myself to be photographed and videoed (including audio recordings) while participating in any EK Dance Academy activities such as but not limited to classes, concerts, exams and competitions. I give consent for EK Dance Academy to utilise these images, videos and other recordings for promotions and publicity, including but not limited to website gallery and social media sites.

Information and Privacy
All the information provided to EK Dance Academy are true and correct, I will notify EK Dance Academy of any changes as required. Failing to do so may affect my child's/my participation in studio activities.
I understand that EK Dance Academy collects information in accordance with the Privacy Amendment (Private Sector) Act 2002. Information provided will be used in matters relating to dance. However, if there is a related secondary purpose to the information provided, and if it is reasonably expected, EK Dance Academy will disclose the specific information requested.
I understand that EK Dance Academy may utilise information collected to be made available to third parties such as insurers, lawyers and accountants.

First Aid
I authorise EK Dance Academy, their staff, representatives and coaches to provide first aid treatment; any effort to prevent further injuries and stabilise my child/myself should it be required. I authorise EK Dance Academy, their staff, representatives and coaches to follow the Australian First Aid Guidelines and to call for an ambulance for my child/myself should it be required. I understand that any costs incurred as a result of emergency medical attention will NOT be absorbed by EK Dance Academy, thus, will have to be payable by me.

Illness, Injury, Epidemic, Pandemic

I understand that in the event my child is sick, injured or misses class due to a holiday, no credit, refund, or make-up class will be issued. If an epidemic or pandemic affects the normal running of classes at the studio, no credit, refund, or make-up classes will be issued, unless decided otherwise out of good will from the Studio Principal.
