I grant Abigail Patterson and Cross-Tied Ministries, its employees, agents and assigns, permission to make, reuse and/or publish photographic pictures or videography of the above student for purposes related to Cross-Tied Ministries and its advertisements, publications, website, social media, and/or publicity, without the right to inspect or approve the finished photograph.
WITHDRAWALS - If I choose to withdraw my child from classes, I understand:
- I am responsible for tuition in full until notice is given in writing to Abigail Patterson.
- Tuition is due for the month in which withdrawal notice is given.
- To receive my child’s costume, I must pay the remaining year’s tuition.
• ACCOUNTS – I understand:
- Failure to keep account up to date (zero balance for each month) will result in the student being suspended from class until account has been paid in full. It is due the 25th of the month before instruction and is considered delinquent if not paid by the 1st. If no payment has been made by the 1st of the month, students will not be allowed to participate in class.
- There are no refunds given on tuition, or additional fees.
COSTUMES* - I understand that in order to receive the Spring Recital costume, my account must be paid through May with a zero balance (including late fees). *Pertaining to students enrolled in Aug. 2023-May 2024 only.
I understand all payments are non-transferrable and non-refundable on tuition, performances fees, or costumes.
I understand that my child must follow the rules for wearing make-up on stage when required.

I agree to read the Cross-Tied Ministries School of Dance handbook and agree that my
child(ren) and I will adhere to all the rules and requirements stated therein.
By signing below, I do hereby release Abigail Patterson, Cross-Tied Ministries, and their agents or representatives of liability for my child of any injury and/or illness/sickness to my child in class, while on the school campus, or while participating in Cross-Tied Ministries School of Dance sponsored performances. I understand that in the event medical intervention is needed, every attempt will be made to contact the person(s) listed on the student’s registration form. In the event that I or next of kin cannot be contacted for the health and well-being of my child, I hereby authorize Abigail Patterson to authorize whatever medical treatment that might be necessary in an emergency situation. I understand that I and my medical insurance carrier are financially responsible for any medical treatment extended to my child, and that Abigail Patterson, Cross-Tied Ministries, and their agents or representatives cannot be held accountable or liable for such medical treatment.
