Waiver of Liability
I/We, on behalf of myself/ourselves and my/our minor child, hereby release Eau Claire School of Dance, it’s officers, agents, and employees from all claims for injury which may be sustained by my/our child while participating in dance classes, rehearsals, performances, and other dance related activities. I/We also release the recital venue chosen by ECSOD from all claims for injury, which may be sustained by my/our child while participating in the Eau Claire School of Dance dress rehearsals and performances.
I/We, also hereby grant Eau Claire School of Dance the right to take, use, reproduce, and publish photographs (that may include images of my child(ren)) for the purposes of advertising or marketing ECSOD’s business or activities. I reserve the right to revoke this permission, but I agree that such revocation will apply prospectively to photographic images taken, used, reproduced, or published AFTER the date of revocation and NOT retrospectively to photographs taken, used, reproduced, or published BEFORE the date of revocation.

I have read and agree to the 2024-25 ECSOD Tuition Schedule.

I have read and agree to the 2024-25 ECSOD Costume Information.

I have read and agree to the 2024-25 Show Package and understand two payments of $35 will automatically be charged on 2/15/25 and 4/15/25.

I agree to have the Eau Claire School of Dance take installments #1-10, costume deposit, winter costume fee, costume balance, show package, and membership fee automatically from my account. I understand and agree that none of the above is refundable. All charges are non-refundable. If not paid prior to the due date, All credit cards will be charged on the 15th of each month, August-April and May 1st. I give ECSOD permission to automatically charge the card placed on file for the above items. If you prefer to avoid the processing fee, payments must be made PRIOR to the due date by cash or check. All cc/bank cards will carry a processing fee for use.

Processing fee of 3.02% and 1.0% will be applied when using cc and bank cards.

Late fee of $25 will be applied 5 days after payment due has not been received.

I/We hereby waive, release and discharge The Eau Claire School of Dance and all divisions thereof of any and all liability and responsibility for injuries, illness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may participate.

I/We waive, release, and discharge from and and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from events, the following entities or persons: The Eau Claire School of Dance and/or their directors, officers, managers, employees, volunteers, representatives and agents, the activity or event holders, activity or event sponsors, activity or event volunteers.

I/We indemnify, hold harmless and promise not to sue the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in the is activity and events, whether caused by the negligence of release or otherwise.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

Summer 2024
Six week session classes will have a one time payment due on June 8th.
All Summer Camps, ballet and Point intensive, Acro will be due at registration.

The Eau Claire School of Dance will provide dance education through any closures that are mandated by local or national government through the means available by ECSOD, Zoom, Facebook, video. Classes will be considered full price and non refundable.
