STUDIO POLICY & PROCEDURE FORM
______ TUITION DUE DATE & LATE FEE: Tuition is due on the 1st of each month for the following month. I understand that the debit/credit card transaction will occur on the 1st for all enrolled classes. All drops or withdrawal requests must be received in writing by the 15th of the preceding month, and no refunds are given once payment has been received. Tuition is non-transferrable. If, for whatever reason, we have not received payment by the 5th of the month, a late fee of $25.00 will be assessed.
______ I AGREE to pay my Monthly Tuition and any additional fees such as Company Fees, Costumes, Competition Fees, Merchandise, Private Instruction, Solos, Duo, Trios through the following payment option (please check) AUTOPAY , CASH/CHECK , CREDIT/DEBIT CARD # If you choose to pay by Check or Debit/Card you are aware that all tuition and additional charges are to be paid no later than the 5th of that month. All payments received after the 5th of the month will be charged a $25.00 late fee. I understand that when a separate charge is due I will need to pay for that either on or before the due date. By choosing Autopay you have agreed and authorized Studio 65 Dance Company to have all tuition charged on the 1st of every month and any additional charges on the scheduled due date. If your card expires please contact the front desk with updated payment information immediately. Any returned checks or decline in payment will be subject to a $35.00 NSF/Return fee.
______ THERE ARE NO REFUNDS OR CREDITS given for classes missed by the dancer or when the studio is closed for holiday breaks. In case of injury or prolonged illness the Dancer is still required to attend classes and observe unless otherwise instructed by a physician.
______ A WRITTEN DROP/WITHDRAWAL/TRANSFER REQUEST must be filled out for any desired changes, and given to the office staff by the 20th. This is to help ensure accuracy, as families are responsible for all enrolled class fees regardless of attendance. If Withdrawal form is given after the 20th you will be charged for the following month.
______ MAKE-UP classes are available for currently enrolled students whose tuition and other studio payments are current. Make-ups must be taken within 4 weeks of class(s) missed, are subject to space availability. Please see front desk to schedule all make up classes. There are no make-ups for a missed make-up. No refunds will be given if your dancer cannot attend make-up classes. Make-up classes may not be used as a substitute for actual enrollment in any given class.
______ COMPANY DANCER AND PARENTS are responsible to know all dates of competitions and other events on the master event calendar. Dancers must be available to attend all competition; Failure to do so may result in dismissal from all company teams. Dancers are not allowed to miss any Required Classes the week before Competition, failure to attend Company Team class that week may result in being removed from the competition that weekend and possibly from the team.
________ COMPANY FEE per Dancer is $300.00 per year and will be charged to all dancers who participate on our company teams. Fees will be charged monthly at $30.00 per month, per student starting in September through June. Or, you may pay at time of registration and receive a 10% discount. Sibling discounts do not apply to Company Fee’s. ______ DRESS CODE must be adhered to at all times, including hair. Failure to follow dress code may result in dancer sitting out of class. If failure to follow dress code dancer will be given one warning and after that will be given proper dress code attire and will be charged for the attire given. ______ APPROPRIATE BEHAVIOR is expected at all times, towards the Studio 65 staff, students, parents and any guests of the studio. Studio 65 Dance Company Inc. reserves the right to dismiss or refuse service to those not adhering to this policy. Students who do not follow the rules or are disruptive in class will receive a warning a verbal 1st time and if the behavior continue we will request a meeting with the parent and student.
______ PARENTS' RESPONSIBILITY TO BE AWARE OF DATES AND EVENTS/ FEES: It is the responsibility of the student’s, parent/guardian to be aware of the Studios scheduled events, recitals, competitions, master classes, etc. along with any deposits or fees related to any event the student participates in. Please be advised that event dates are subject to change or cancellation at the discretion of the Studio.
_____ END OF THE YEAR SHOWCASE will be held at the end of the dance season in June. Parents need to fill out an OPT OUT form and give to the front desk no later then March 1st if their dancer is has a conflict and will not be able participate. If OPT OUT form is not received by March 1St parent will still be responsible for Showcase Costume charges and will not receive a refund. All Showcase costumes will be charged the week of March 11th for all participating students. We highly recommend that all dancers participate in the Showcase and are mandatory for all Competition Dancers.
______ CHILD PICK-UP: Please pick up your child after their class or classes have been completed. Please do not leave your child at Studio 65 Dance Company Inc. any longer than 15 minutes after their class schedule has finished.
By Signing below, I have read, acknowledged and initialed the above policy and procedures. I understand that my child’s participation is at will and I, hereby agree to hold harmless and release Studio 65 Dance Company Inc. from all liability or injury that may occur as it relates to my child’s participation in classes and events.
