Sessional Classes: Full tuition is due at the time of the registration.
Full Season: Registration Fee is due at the time of registration and Tuition Fees are due on the 1st of each month from September to June.
Fees may be paid by Visa, MasterCard, Amex or Cheque, however, if paying by cheque the customer agrees to have a valid credit card on file.
Prior the Start of the Season/Session: You may cancel registration any time within 7 days after registration.
Once the Season/Session have started: Notice must be received by the 15th of the month. After the 15th of the month will have tuition fees charged for the subsequent month. In that case, student is welcome to attend class for that period.
All cancellations must be done in writing and delivered in person, by mail, or email. Please note that Registration Fees are non-refundable.
WAIVERS AND RELEASES
Photo/Image: I give consent for images (photographs, video) of my dependent to be taken and used to document the activities of Class Act Studios. I grant Class Act Studios permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent to be used, I will indicate this in writing and the signed letter will be attached to this document.
Medical: I give my permission for Class Act Studios, Studio Teacher, or Studio Parent to take my child to a medical/dental facility, if necessary. In case of emergency, if none of the above can be contacted, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being of my dependent. I accept full responsibility for all costs of said medical care and any emergency treatments. Class Act Studios will not be responsible for the cost of any medical care or emergency treatments.
I hereby waive all claims whatsoever in connection with such medical treatments.
Physical Activity Waver: I agree that if my dependent or I (heretofore known as "we") engage in any physical exercise, class, or activity, or facility on the premises or any venue where we participate as representatives of Class Act Studios, we do so at our own risk. I agree that we are voluntarily participating in activities and use of said facilities, premises (including the parking lot) and designated Class Act Studios venues. I assume all risk of injury, illness, damage, or loss to us or our property that might result, including, without limitation, any loss or theft of any personal property. I agree that this consent and assumption of risk statement covers each and every event or activity sponsored by Class Act Studios. I agree to release and discharge you (and your affiliates, employees, agents, representatives, successors, and assigns) from any and all claims or cause of action (known or unknown) arising out of your negligence. I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability. I am waiving any right that I may have to bring legal action to assert a claim against you for your negligence.
THE STUDENT AND GUARDIAN AGREE TO THE FOLLOWING:
The Studio reserves the right to combine or cancel any class and that the teachers reserve the right to move my child to a different level if the one they are registered in is not the correct level.
The Studio is a Nut/Peanut Free Facility.
I am aware the Studio has a dress code (available on the website) and failure to comply may lead the dancer to be excused from class.
Students should arrive at least 10-15 minutes prior the start of their class and should be picked up promptly after class. Students should wait for pickup inside the building and not in the parking lot/sidewalk.
Absences must be communicated to the instructors or the Studio Office.
Coats must be properly hung and shoes/boots must be properly placed on the shoe racks provided. No outdoor footwear must be worn past the mud-room/shoe rack area.
Backpacks must not be left in the hallways and should be taken into class with the student.
Absolutely no food or drinks (except water) are allowed in the dance studios
No gum chewing in class.
Noise and running will not be tolerated in the hallway and waiting areas.
Students are encouraged to bring their own (labeled) water bottles for class. Water bottles must not be left at the Studio after class is done as they will be discarded. All Lost & Found items will be donated at the end of each month.
All students must take full responsibility for any personal items brought to the studio. The staff at Class Act Studios will not be responsible for any lost or stolen items.
Students are expected to treat their instructors and Studio Staff with the utmost respect, as they, in turn, will do the same to you.
* * * APPLICABLE TO FULL SEASON STUDENTS ONLY * * *
A Costume Deposit of $100.00 per costume is due October 1st and are non-refundable after October 1st. Costume balance (including shipping and duties) is approximately $15-$30 (may be $50+ for ballet intermediate and senior students or professional tutus). Costume balances are due March 1st and shall be applied to the credit card on file. Costumes are considered final sale after October 1st and any cancellations after that date will be charged a flat fee of $30.00 per costume.
A Recital Fee of $50.00 is due on October 1st and charged to all students participating in the Year-End Performance in June. Recital Fees help to offset the costs with the production of the show, theatre rental, programs, technicians, staff, etc. Recital Fees are non-refundable after October 1st. The Annual Year-End Performance is held in June. Recital tickets are approximate $25.00 (plus applicable taxes and service fees) and everyone wanting to watch the performance must purchase a ticket.
Due on September 1st and charged to all students enrolled in ballet, jazz, and tap examination classes who will be taking an exam. he fee will be comprised by Entry Fee amount required by the institutions (RAD/ADAPT) for your child's examination level plus an additional fee charged by the Studio to cover expenses with examiner's travelling, accommodations, food, mock exams, etc. Examination Fees are approximate $120-$150 for jazz and tap examinations (depending on the level taken), $130-$165 for graded ballet examinations (Grades 1 to 8), and up to $400 for vocational ballet examinations (Intermediate Foundation to Advanced 2). Examination fees are non-refundable after September 1st.
Due on November 1st and charged to all students enrolled in competitive routines. Information on competitive routines and competitions the Studio will be participating is released in September. Competition Fees are approximate $185 for groups, $211 for duos/trios, and $340 for solos. Competition Fees are non-refundable after November 1st.
Due December 1st and charged only to students enrolled in competitive solos, duos, and trios, choreography fees will cover 3 hours of instruction for the creation of the routine. Once the piece has been created, students are responsible for practices.
I understand and agree with the terms and conditions described in the Competition and Examination pages available in the Member Area portion of the website.
All Examination students (Ballet, Jazz, and Tap) are required to attend the Examination Intensive Week Workshop from August 26-29.
