I have fully read and understand the policies of Villa Dance Company. I agree to let my child participate in all of the events that the studio may attend and to follow the guidelines explained in the policies (including recital). I understand that by placing my child in this program it obligates myself to certain financial obligations, as explained in the information. I agree to pay the fees listed. I understand that any money, fees or payments are non-refundable, no exceptions. I also agree to let my child be under the instruction of all Villa Dance Company staff. In these unprecedented times I understand classes may need to be held in a different format and financial obligations will still be in place for the alternative instruction. I will not hold Villa Dance Company or its affiliates accountable for any lost, stolen, damaged items or injuries at any time. I agree to have things that may get left behind picked up in a timely manner or I forfeit them to the studio.

I have seen the financial information and understand my financial responsibilities. I am aware that by enrolling my child(ren) at Villa Dance Company that I am required to purchase items listed on the financial information sheet. I understand that an occasional prop or accessory is needed and cannot be foreseen this early in the season. I agree to the fees and costs listed, understanding that prices are subject to change on shoes and clothing as determined by the manufacturer. I agree to pay these fees at the required time. I also understand that by not paying these fees by the deadline a late fee will be applied. I do realize that if we drop classes I am obligated to pay any occurred balance, including unpaid costumes. I also understand that until I have filled out a drop form, I will be obligated to pay monthly tuition. I understand that all money paid is non-refundable.

I hereby represent to Villa Dance Company that my child is of sound health and has no history of a medical or physical condition which could place my child at risk because of a said condition. I also hereby acknowledge that I have been informed by Villa Dance Company of the nature of instruction that my child will receive and that such instruction involves physical exercise and stress, which could result in and injury or illness. It is further agreed that unless caused by the negligence of the studio or staff, we hereby waive all claims and hold Villa Dance Company or its staff harmless for any such injury or illness incurred during the course of the instruction, while at events, or appearances. I also agree that Villa Dance Company and staff will not be held responsible for any family member or friend that gets injured or becomes ill while at the studio or event that may take place. I give my permission to transport my child to the nearest hospital in case of an emergency. I further grant permission to Villa Dance Company to use any photos/videos of my child for any advertising or legitimate purpose.  I agree to all of the above and agree to adhere to all policies and financial obligations. I also confirm that I have correctly filled out all of the registration information so the staff has my information should a situation arise.
