Rising Stars School of Dance is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific exercise on a medical condition. I attest that I am in good physical condition and able to participate in weekly classes.

I understand that when participating in any exercise or dance program, there is the possibility of physical injury. If I engage in any of Rising Stars dance classes, activities, and/ or annual recital I agree that I do so at my own risk, are voluntarily participating in these activities, assume all risk of injury to myself, and agree to release and discharge Rising Stars School of Dance from any and all claims or causes of action, known or unknown, arising out of Rising Stars School of Dance's negligence.

I attest that I have read all rules, regulations, and policies of Rising Stars School of Dance and agree to abide by them. I understand failure to abide by any rules, regulations, and policies may result in dismissal from Rising Stars School of Dance classes/ activities/ annual recital.

I agree to pay tuition, costume payments, and/ or additional fees on time and in full throughout the dance season. I understand that failure to do so may result in dismissal from Rising Stars School of Dance classes/ activities/ annual recital.

I give my consent for images (photographs, video) to be taken and used to document the activities of Rising Stars School of Dance. I grant Rising Stars School of Dance permission to use the images for educational and promotional purposes, online or in print.

I understand that the Policies and Procedures are subject to change throughout the dance season and it is my responsibility to adhere to all new or edited policies.

I agree to pay tuition and/ or additional fees on time and in full throughout the dance season. I understand that failure to do so may result in dismissal from Rising Stars School of Dance classes/ activities/ annual recital. I understand that if I am no longer interested in taking classes at Rising Stars, I must send written notice within 2 weeks of the last intended class. I understand that normal charges within this two week period must still be paid in full. I understand that failure to communicate intended withdrawal from classes will result in continued payments. I agree to the withdrawal policy as outlined in the Rising Stars Policies & Procedures handbook.
