I understand that credit card Auto-Pay is required and that tuition is based on a 4-week billing cycle and is due in advance by the 25th of each month. I understand that classes do not have a start date (unless specified in class name description). After I register for a class, my card will be charged full tuition and is only prorated if there are 2 or less lessons left in the month. After this, regular tuition will be charged on the 25th of each month and will apply to the upcoming month. I understand that some months may not have 4 full lessons due to holidays, tuition will not be reimbursed. Most of the time these are made up with months that have 5 lessons or with the 2 shows a year which are free.

I understand that a 3% administration fee for credit card, 1% administration fee for ACH (Echeck) is applied to each tuition payment to cover processing costs.

I understand that these classes do not have an end date and will continue even after performances. IT IS PARENTS RESPONSIBILITY to log into the Parent Portal and remove student from class if they wish to stop.

LEGACY DANCE
AGREEMENT AND RELEASE OF LIABILITY
I hereby acknowledge that I have voluntarily permitted my registered child (enrolee) to participate in dance and tumbling instruction, training and/or activities at Legacy Dance, LLC and Legacy Dance Complex.
I am aware that dance and tumbling instruction and training involve hazardous activities, and I
am voluntarily permitting enrollee to participate in these activities with knowledge of the danger
involved and hereby agree to accept any and all risks of injury to enrollee that may result there from.
In exchange for Enrollee being permitted by Legacy Dance, LLC to participate in these activities
and use their facilities, I hereby agree that I will not make a claim against or sue Legacy Dance, LLC or
Legacy Dance Complex or any of its principals, employees or agents, for injury or damage resulting
from negligence or other acts, whosoever caused, by any employee or agent of Legacy Dance LLC as
result of Enrollee’s participation in dance and gymnastics instruction, training or activities.
In addition, I hereby acknowledge and agree to release, defend, indemnify and hold harmless
Legacy Dance LLC and Legacy Dance Complex, it’s principals, employees and agents, and to assume
full responsibility for any loss or damage for any claim, lawsuit or demand for loss or damage, on
account of injury to Enrollee, whether caused by the active, passive or sole negligence of Legacy Dance
LLC, it’s employees or agents while Enrollee is on the property of Legacy Dance LLC and Legacy
Dance Complex, or is participating in any way in any dance and tumbling instruction, training or
activity.

Should it be necessary, in the opinion of a member of the coaching staff of Legacy Dance LLC,
to render first aid and assistance to Enrollee, I hereby grant permission to the coaching staff of Legacy Dance LLC, to render such aid and assistance.
I have carefully read this agreement and release of liability and fully understand its contents.

Financial Agreement
I have read, understand and comply with the policies regarding tuition including due dates, late fees and
the four week cycle.
Terms: C.O.D. Unless previous arrangements have been made. All accounts aged over 30 days may be
charged a finance charge of 1.5% per month (18% annually). Customer accounts remaining unpaid after 30 days may be subject to attorney’s fees, filing fees, court costs, and all collection costs (up to 50%
added principal balance) associated with the ultimate collection of the unpaid principal balance, with or without suit. A return check charge of $20.00 will be charged on all returned checks. I agree to these terms and conditions by checking below.

Photo/Video Release
Legacy Dance LLC. Is hereby granted permission to take photographs/video of the students to use in
print and media advertising, posters, and other promotional materials. Permission is also granted to
post photos on Legacy Dance website, Instagram and Facebook page. Legacy Dance assures that all
photos/videos posted will be age-appropriate and modest.

I hereby acknowledge that I have voluntarily permitted my child to participate in dance and tumbling instruction, training and/or activities at Legacy Dance, LLC and Legacy Dance Complex.
