FALL SESSION I & Session II:

CALENDAR: There are 35 classes for the full session (September 10th, 2020 to June 20th, 2021), broken into two (2) semesters.
• Semester 1: 18 classes in (September 10th, 2020 to January 30th, 2021)
• Semester 2: 17 classes in (January 31st, 2021 to June 20th, 2021).
The class schedule may change slightly between the two (2) semesters based on the development of COVID-19, but your classes will still fall on the same days, likely at the same or very similar time.
At registration of each semester, the full semester’s tuition will be due unless you are enrolled in autopay and for a payment plan. Payment plan options include a payment for the first month and deposit for the last month, and monthly increments after. If you would like to discuss payment options, please reach out to Studio E.
• Payment plans will only be available through autopay or checks.
• If you would like to pay by check, this can be arranged as well, pre-written, post-dated checks.
o As mentioned, the first check will be for the first (i.e., September) and last month deposit (i.e., January), the second check will be for the next month (i.e., October), etc.
TUITION DISCOUNT: Those that would like to pay in full for the fall through spring session will receive a 5% discount if paid through Zelle, Quickpay or Cash. This is payable in two installments: 50% paid on registration, with the balance due by November 1st. There will be no refunds given after your first class.
A ONE-MONTH TRIAL PERIOD IS GIVEN TO ALL NEW STUDENTS. If at the end of one month a student is dropped from the register, the deposit will be refunded minus a $30 cancellation fee. If the student is dropped from the register at any other time during the session, the deposit is forfeited.

STUDENTS ENROLLED MUST ABIDE BY THE RULES OF STUDIO E WHICH INCLUDE

1. Being prepared for class ON TIME in the designated uniform, hair tied back, and jewelry off in classes, clean sneakers in all Hip-Hop classes.
2. Never using cell phones to call or text in the dance rooms.
3. Disrespecting and instructor or staff member will result in consequences dependent on the severity of actions, which may result in termination/suspension from Studio E.
4. Students must arrive on time - students arriving late may be asked to sit out of class (this will be done at the discretion of the instructor).
5. Students registered in advanced or intermediate classes must take all pre and/or co-requisites designated for the class (ex: must take 3 additional hours of ballet to be in a pointe class).
6. Excessive absences may result in being taken out of a section or out of the entire dance (this will be done at the discretion of the instructor).
7. If there are less than 3 students in the class, the instructor has the right to shorten the class to 45 minutes. If there is only 1 student in the class, the instructor has the right to shorten the class to a half hour.
8. If a mistake was made in the calculation of your tuition, you agree to pay the difference.
NEW COVID-19 SAFETY and TOUCHLESS RULES:
• Arrival: Upon Arrival all staff and students will have a hand sanitizing station where they will sanitize and will have their temperature checked.
- Students are to come to the studio dressed and ready to dance. They will bring their bags into the studio with them. They will place their dance bag either in from of their box by the mirror or behind their box by the barre.
• Waiting Room: The waiting room is to remain empty. Parents must drop off and pick up their children in a timely manner at the door in order to adhere to social distancing standards. They cannot wait inside.
• Cleaning: Dance spaces and touched surfaces will be sanitized and wiped down with a disinfectant in between classes.
• Props: There will no use of shared props during dance class
• Across the Floor: There will be no across the floor during dance classes unless it is safely socially distanced.
• Masks: Masks will be required for all instructors as well as students ages 2+ during class.
• COVID Screening: Instructors will be required to complete a health questionnaire each week prior to teaching.
• Social Distancing: There will be no physical interaction between instructors and students.
• Students have a 5 ft wide by 6 ft long box taped onto the floor and are limiting in-person students to 9 or 10 students per class, in addition to our virtual classes.
• Hand Washing: Upon arrival to the studio, as well as in between each class, the instructor must wash hands and/or use hand sanitizer.
• Zoom: All in person classes will also have a virtual option. There will be a 40-inch TV in class that will run Zoom and display the student’s videos that will be taking class virtually.
• Make-ups: Make-ups can only be taken via Zoom, there is absolutely no credit given for classes missed due to limited space due to COVID
• By participating in Studio E’s in person classes, I/we have read through the new safety practices located at studioE.net/covid/fresh-meadows/. Please make sure to check this page periodically for updates. 
I/we understand that by participating in Studio E’s in person dance classes there is a chance that class may need to move to virtual classes through Zoom. This may result from a Stay at Home / Quarantine order from government officials. There will be no refunds / credits given if Studio E needs to transition to virtual classes.

PERFORMANCE CLASSES

All students have the opportunity to perform in one of our ends of the year Showcases, except for our open Adult classes and Tiny Dancers. Anyone registered for a class that is part of our Senior Showcase will be required to be part of the performance and MUST attend the Tech and dress rehearsal or they may not be allowed perform in the show. Anyone registered for a class that is part of our Junior Recital will have the option to opt out of the performance. Any student opting into a performance class must pay all fees associated with it such as, but not limited to costumes, tickets, and performance fees. A $50 deposit per costume and performance fees for the Sr. Showcase and a $40 deposit for the Jr. Recital.
• If we are allowed to have an in-theater performance, our goal is to have a recital scheduled for June 13th

ABSENCES & MAKE-UP CLASSES

WE DO NOT GIVE REFUNDS OR CREDIT FOR ABSENCES. There will be NO CREDIT given for illness or vacations. If you are unable to attend class, you can arrange to take a make-up class virtually. This is solely your responsibility. Make-up classes are valid only while you are registered in the studio, and during the same the session you are taking the classes. For each missed class, you can only take one class of equal or less hours. To avoid overcrowding classes, please call or email to arrange when you will be taking a make-up class. In case of snow or extreme emergency days causing Studio E to close you will be entitled to a make-up class - no refunds will be made. There will be no makeups permitted in performance classes after April 30th unless approved by the teacher. If you schedule a make-up class and do not show up, you will forfeit the makeup

LATE FEE

Tuition is due the first week of each month. Any tuition received after the 14th of the month will be charged a $15 late fee. For each additional bill sent, an additional $10 fee will be charged. If you are late two times, you will be required to sign up for autopay.

Merchandise for purchase is located at our studio and will happily accommodate all of your dancewear needs. All payments must be through ZELLE, QUICKPAY, CASH, OR CHECK ONLY.

I/we (the dancer and/or parent/guardian) understand that registration in a class at Studio E constitutes consent for public use of pictures and videos taken of my child (or me) at the studio, on Zoom, or on stage without limitations as to time or frequency of use, for any or all of the following purposes:
1. Newspaper release
2. Education, instructional, or teaching purposes
3. Publicity or fundraising
4. Advertising or social media

LIABILITY

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By agreeing to this waiver, I/we assume all risks related to the use of any and all spaces used by Studio E. In the event of an injury or emergency, if I am unable to be contacted, I give permission for Studio E to obtain medical services for me/my child.
I/we realize that participation in dance classes that could take place via online dance classes on Zoom. I/we are engaging in this activity at our own risk it is up to me/us to perform these activities in an open area free from obstacles. Go at my/our own pace. By participating in these online classes, I/we agree to waive all liability and hold harmless Studio E for any injury to myself or to others, or property damage. I/we agree to participate in these classes at our own risk.
I/we agree to release and hold harmless Studio E, including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Studio E liable for any personal injury or personal property damage, which may occur on the premises before, during, or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Studio E.
I agree to be in the lobby area at the time my child's class ends. My child is in good health to be participating in the activity of dance. I understand the above policies of Studio E is for the well-being of myself or child and the Studio E staff and agree to abide by them.
WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
ASSUMPTION OF RISK / WAIVER LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate on behalf of Studio E, School of Dance and related events and activities, the undersigned acknowledges, appreciates and agrees that
1. Participation includes possible exposure to and illness from infection diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIFENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation, and bring such top the attention to the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, and personal representatives and next of kin, HEARBY RELEASE AND HOLD HARMLESS Studio E, School of Dance) their officers, officials, agents and/or employees, premises used to conduct the event (“RELEASEESâ€), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss of damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
