As the legal parent or guardian, I release and hold harmless Center Stage School of Dance, its owners and operators any and all claims, demands and causes of action whatsoever, arising out of related loss, damage or injury or death that my be sustained by participating and/or the undersigned while upon the premises or any premises under supervision of Center Stage, its owners and operators in or on route to and from said premises. I understand the physical nature of dance and accept all responsibility for all actions while my child participates in. I acknowledge that it the studio is forced to close for safety reasons all classes will be moved to an on line format at the same day and time. Virtual classes will commence within a week of any government announced closures. I hereby grant Center Stage School of Dance the irrevocable right and unrestricted right to use, reproduce and publish photographs of my child for editorial, trade, and advertising. Refund will only be issued within the first 4 weeks of classes. A written letter of withdrawal must be submitted to Center Stage to obtain a refund. There are no refunds on Costume deposits or registration fees A $25 registration fee will be applied to each family at the time of registration. All classes will be charges a $50 costume deposit at the time of registration. Costume deposits are not refundable after November 15th 10% late fees apples to any account not settled before the start of the next season.
