School Policy:
Agreeing to the terms and conditions will indicate my consent to have my child participate in the Maple Academy of Dance Inc. (MAD Inc.). I hereby release the Academy from all claims for damages to person or property from any accident or injury, which is caused or arises from participation at the MAD Inc. I am aware there will be zero tolerance for poor behaviour and immediate dismissal will result for any person not following the values embodied by the MAD Inc. My child(ren) and I comply with the COVID-19 safe policy and protocols.

Cancellation Policy:
All costume deposits, registration fees, initial, full payments and monthly payments are not refundable or transferable. No refunds will be given for cancellation or for payments that have been processed. No refunds are given for competition entry fees.

The last day to change classes for full season programs is November 1st and notification in writing must be given to the Director by that date. After November 1st your account will be charged a $25.00 administration fee for any changes made to your classes.

The last day to cancel for the season is December 15th and notification in writing must be given to the Director by that date or your account will be charged a $150.00 cancelation fee. Please ask to fill out the cancellation request form and return to the office by email.

Natural Disaster or matter outside of our control:
In the event that classes must be cancelled due to matters outside our control (e.g. building closure, natural disaster, virus outbreak, government enforced action), dance families will be contacted by MAD. Refunds will not be given for missed classes that do not occur because of reasons outside of our control. Makeup classes or online lessons may be offered to replace missed classes however this cannot be guaranteed. If we cannot offer in person classes due to reasons outside of our control, our season will continue on Zoom.

Hybrid Model:
Due to the current COVID-19 pandemic, the number of students permitted in the studio is limited. When a class fills, others may join to take the class from home via Zoom. Students who are required to isolate may also continue to take classes online. Should the studio be forced to close the physical building, all programs will continue to run on Zoom. There are no refunds for missed classes.

Placement:
In the event you register for a class that is not age or level appropriate, MAD reserves the right to change your placement and will notify you of the move.

Email:
By agreeing to these terms and conditions you give Maple Academy of Dance Inc. permission to send newsletters and other correspondence through MailChimp and email.

Photography:
All pictures and videos affiliated with MAD Inc. are property of the Academy and may be used for advertising without written consent or any compensation whatsoever.

Auto-Pay:
All students need to enrol in Auto-Pay on the parent portal before they can complete their registration. Class tuition will automatically be processed on the 1st of each month. A change in credit card information must be updated before the 1st of the month to avoid late charges.

Declined Auto-Pay Payments:
Accounts with a declined Auto-Pay payment will be charged $5.00. In addition, a late fee charge of 10% will be processed should payment not be received by the 10th of the month. If two or more of your Auto-Pay payments are declined, the late fee will be charged, and you will be required to pay ahead. The credit card on file will be used for this charge.

If paying by cheque, all post-dated cheques must be received at the front desk and post-dated for the 1st of every month. The first cheque must include registration fee and first monthly payment. The second cheque dated October 1st must also include the $70 costume deposit per class, and all other post-dated cheques from December until May must be dated the 1st. NSF cheques will be subject to a $35.00 service charge. A second NSF cheque will result in cash only payments.

If you wish to pay by cash or debit we must have the first and last month payment at time of registration.

**Participation in any MAD event or class will be suspended along with recital costume withheld until your account is paid in full.

Additional Terms:
The Academy reserves the right to cancel classes for any reason it feels is appropriate without any financial compensation. MAD Inc. is based on a 31 week, 9-month plan. Payments are the same each month regardless of how many weeks fall in that month. No refunds will be given for missed classes and classes that may be cancelled due to poor weather conditions.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19:
Maple Academy of Dance Inc. (the Academy) has put in place preventative measures to reduce the spread of COVID-19; however, the Academy cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Academy could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Academy and that such exposure or infection may result in personal injury, illness, permanent disability, and possible death. I understand the risk of becoming exposed to or infected by COVID-19 at the Academy and I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability and death), illness, damage, loss, claim, liability or expense of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Academy or participation in Maple Academy of Dance Inc. (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Maple Academy of Dance Inc. or its employees of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Maple Academy of Dance Inc. its employees, agents and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Maple Academy of Dance Inc. program.

Preventative Measures:
• Reduced class sizes
• Students will be dancing in their own 6’x6’ dance space
• Masks are worn upon entrance and exit, and in common areas
• Outdoor shoes will be removed upon entry
• Screening and temperature checks
• Temporarily closed change rooms
• Enhanced cleaning and sanitization
• Frequent hand washing
• Frequent cleaning of touch points and bathrooms throughout the day
• Morning and evening disinfecting with Vital Oxide (Health Canada approved disinfectant against COVID-19) - safe for children, animals and planet.
