CARD HOLDER AGREEMENT AND AUTOMATIC PAYMENT AUTHORIZATION: Being the authorized cardholder or the Corporate Officer, by agreeing below I understand to the terms set forth in this agreement, agree to pay, and specifically authorize to automatically charge my credit card for the services provided. I further agree that in the event my debit/credit card becomes invalid, I will provide a new valid debit/credit card upon request, to be charged for the payment of any outstanding balances owed. I furthermore confirm that I understand that there are no refunds, transfers or credits of any kind, and that my obligation is monthly and can be cancelled any time before the 22nd of each month, for the following months tuition. I also waive my right to "chargeback" a payment because I miss the monthly withdraw deadline, and authorize my bank to cancel any "chargeback" initiated upon receipt of this acknowledged waiver. I hereby authorize Glass House Dance to charge the balance of my account automatically on the 22nd of each month, for the following months tuition. I hereby agree that this arrangement will be continuous until I provide written notification via email to: info@glasshousedance.com discontinuing enrollment.

After the 1st of the month, balances are considered past due and will be charged a 20% late fee. After the 7th of the month, my dancer will be automatically withdrawn, until the balance is paid in full.

I hereby warrant that I am of legal age and have the right to contract in my own name and on behalf of the minors in my care. I have read the above Releases and Authorizations, and I am fully familiar and agree with its contents.

GENERAL LIABILITY RELEASE:
By agreeing below, I hereby authorize the faculty and staff of Glass House Dance to act for me according to their best judgment in any emergency requiring medical attention for myself or my enrolled dancer.

I shall defend, indemnify and hold harmless Glass House Dance, its officers, officials, employees and volunteers from and against any and all claims, suits, actions, or liability for injury or death of any person, or for loss or damage to property for any reason while at Glass House Dance, coming to or going from Glass House Dance, or while at any performance, event or function associated with Glass House Dance. I certify my acceptance of the above statement and agree to accept liability for my enrolled dance and for any person(s) associated with my presence at Glass House Dance, coming to or going from Glass House Dance, or while at any performance, event or function associated with Glass House Dance.

I have no knowledge of any physical or mental impairment that would be affected by the student’s participation in the Glass House Dance programs. I certify that any medical information provided for my student(s) is complete and accurate.

I hereby warrant that I am of legal age and have the right to contract in my own name and on behalf of the minors in my care. I have read the above Releases and Authorizations, and I am fully familiar and agree with its contents.

Glass House Dance offers all enrolled students two optional performance opportunities annually. The Informal Performance Student Showcase (December) and the Glass House Ball (June). Participation in each performance is optional, however sign up to participate in each is automatic!

It is assumed that all enrolled students WILL perform & participate in each performance, unless opted out during a specific Opt-Out period. Each Opt-Out period is 3-4 weeks in length and 3 courtesy reminders will be sent during this period, as each Opt-Out deadline approaches.

Additional participation fees do apply to perform in each performance and these fees are automatically charged to enrolled student’s card on file, on specified auto-charge dates. Once charged, no refunds, credits are transfers will be issued as your dancer’s participation has been accounted for.

By signing below, I hereby acknowledge and agree to the “opt out only” performance policy and agree to the automatic participation fee charges. I acknowledge that if I do not wish for my dancer to participate in the Informal Performance Student Showcase (December) or the Glass House Ball, (June) that I must opt them out of performing, (in writing via email) during each specific opt out period.

For good and valuable consideration herein acknowledged as received, I hereby grant to Glass House, its affiliates, legal representatives, and assigns, and those acting with Glass House’s authority and permission, the irrevocable and unrestricted right and permission to create, use, re-use, publish and re-publish video recordings, audio recordings, photographs, or other media that contain or capture my likeness or voice or in which my likeness or voice may be included (the “Images”) the mission and goals of Glass House, including advertisements, brochures, or other promotional materials. The Images may be used with or without my name, and in any and all media now or hereafter known. I acknowledge and agree that Glass House owns all right, title, and interest in and to the “Images”, including all copyrights therein and the full and unrestricted right to edit and modify the “Images”, and I hereby assign and agree to assign any such interest that I may own or control to Glass House. I also consent to the use of any printed matter in conjunction with the “Images”. I hereby waive any right I may have to inspect or approve the “Images” or any finished product or products incorporating the “Images” and any written or other print material that may be used in connection therewith, including print material containing my name. I acknowledge that nothing in this Agreement obligates Glass House or any third party to make any use of the Images.
