Studio Waiver
I understand that there are risks of injury associated with physical activity. I hereby agree to release Dance Dynamics (DD) and hold DD harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will. I hereby agree to indemnify and hold harmless DD and any of their officers, agents, employees, and representatives from any liability or claim or action for bodily injury or damages resulting from or in any way arising out of the participation in any of the programs on or off premise.

I give DD my full and unlimited permission to use mine or my participants’ picture, image, likeness, or video (including retouching, editing, cropping or any other alteration) in or on any form of advertising without compensation. These images may be used in (but not limited to) publications such as emails, brochures, newsletters, magazines, websites, and videos. I waive any right to inspect or approve these products. DD does not administer medication and any medication is the sole responsibility of the individual’s parents or guardian. DD prefers all students to stay in the studio during breaks and asks that if a student leaves, they have parent/guardian permission and are wearing appropriate attire (no leotards, midriff tops, or booty shorts). DD may take participants outside and throughout the Downtown Summerlin mall.

In addition to phone calls and emails, DD uses text as a method of communication. Should you not want DD to text you, please contact the front desk to opt out.

Covid-19 and Disease Waiver: Participants understand that attending in person classes at Dance Dynamics is of their own free will. Participants will abide by all social distancing and sanitation requirements required by DD. Participants will not come into the studio if anyone in the household is sick or has a symptom or a suspected symptom of Covid-19. Nor will participants come on premise for a minimum of 14 days if participants have traveled from areas on the CDC Level 3 Travel Health Notice. DD has implemented recommended protocols and preventative measures recommended by the CDC to help reduce the spread of Covid-19 however DD cannot guarantee that participants will not become infected with Covid-19. By signing this agreement, participants acknowledge the contagious nature of Covid-19 and other diseases and voluntarily assume the risk that participants may be exposed or infected by attending classes at DD. Such exposure or infection may result in personal injury, illness, permanent disability, and death. Participants understand that the risk of becoming exposed to or infected by Covid-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, DD instructors, volunteers, and program participants and their families. Participant voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to participant (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that participant may experience or incur in connection with attendance at DD or DD events. Participant hereby releases, covenant not to sue, discharge, and hold harmless DD, its instructors, agents, owners, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. Participant understands and agrees that this release includes any claims based on the actions, omissions, or negligence of DD, its instructors, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any program with DD.

The undersigned acknowledges and agrees to always comply with DD’s procedures. The participant has read and understood the terms of this assumption of risk, release and waiver of liability, and indemnity agreement and agree to its terms.

1. Parents are welcome in the studio. If you are not staying in the studio, please make sure we have current contact information so that we may call you if there is an issue.
2. If there happens to be a dancer waiting for their next class, parents - please give up your seat.
3. Do not congregate by the window.
4. Park your strollers outside
5. The studio is PEANUT FREE.
a) Dancers can bring food into the lounge and outside. No food allowed in the studio rooms.
b) We do not supply plates, forks, knives etc.
6. Dancers will be released from their classrooms. Please be ready to promptly pick your dancer up from the studio door or outside of the front door of the studio. We cannot control where your dancer goes when they walk outside of the studio.
7. Do not park in the loading zone for longer than 2 minutes. It is a loading and unloading zone only. Do not park there & wait for your dancer to complete their class.
8. Shoes:
a) Do not wear tap shoes in the hallways - it is slippery!
b) Untie tap shoes after they are removed. It makes it so much easier to get them back on!
c) Do not wear street shoes in the studio rooms. Only dance shoes permitted. Hip Hop in particular, you cannot wear your street sneakers in the studios - they should have a separate pair of sneakers for dance classes.
d) You are not permitted to wear socks only in your Team Rehearsals. You must wear the shoes that you are going to wear for that routine. If your routine requires a jazz shoe, wear a jazz shoe to rehearsal.
9. Please have your dancer bring a refillable water bottle to the studio. NO glass bottles permitted. We are not supplying cups for water. If your dancer needs water, they can either buy a bottle from the front desk or they can buy a refillable water bottle. These charges will be applied to your account.
10. Costume deposits are non-refundable. If a costume needs to be tailored due to a growth spurt or wear tear that is at the expense of the parent.
11. Remind your dancers to use the restroom before and after class, not during class. Dancers cannot use the restroom in groups.
12. Wear proper Ballet Attire to Ballet Classes.
a) Black leotard, pink tights, hair in a clean bun with no wispies
b) A Ballet sweater or leg warmers permitted during winter season
c) No leggings, sweats, shorts, hoodies etc. permitted
13. Throw your trash away, do not leave it on the ground, in a cubby, on the couch etc. Clean up after yourself.
14. No wearable electronics.
15. No slime in the studio.
16. No gum allowed in studios.
17. No food or drink on the laptop table.
18. Remind dancers to not pick the paint or the wallpaper off the walls. Do not pick the tint off the windows.
19. Do not gossip.
20. Discrimination of any kind will not be tolerated.
21. No PDA by dancers or parents in the studio.
22. No smoking, vaping, pipes, drugs, drinking alcohol or any illegal or illicit substances are permitted on the premises.
23. Be mindful when handing out birthday invitations within the studio. We understand you cannot invite everyone but keep in mind that when friends do not receive invitations it is hurtful to them, and they do not always understand why they could not be invited.
24. You cannot add additional classes unless your balance is paid in full.
25. No cell phones in class. Dancers should secure cell phone in a locker or leave it with a parent.
26. Parents do not take pictures or video dancers through the window. Teachers will invite you in at appropriate times for pictures and video. Wait for an invitation to record. Sometimes parents are invited in as an audience, and recording is not welcomed. Do not post pictures on social media of other people’s children.
27. We encourage dancers to cover up when exiting the studio. Dance wear is appropriate for the studio, not in public.

29. Tuition is due on the 1st of the month. Late fees are assessed on the 8th. If your account is not paid by the 15th, dancers will be removed from class.
30. Competition team fees will be charged a $20 late fee will be assessed on the 8th day after the due date.
31. Classes will start promptly at their designated start time. If you are going to be 15 minutes or more late to class, schedule a makeup.
32. There is a 30-day cancellation policy. Please let us know in writing if you want to cancel your dancer's membership. You can email your cancellation request to Help@DanceDynamicsLV.com
33. Make sure the front desk is aware of any allergies/medication/physical/mental condition of your dancer. We also need an emergency contact in the file.
34. We encourage Auto Pay.
35. Tuition does not increase in price for months with five weeks, nor does tuition decrease in price for holiday shortened months. Rather than issue fluctuating monthly tuition charges, tuition prices are averaged across the entire dance year. One monthly price, one monthly payment!
36. Log absences in the studio APP or online portal if your dancer will be absent. If your dancer has multiple classes in a day and is only missing one, please still let us know about the absence. Your dancer has 30 days to make up a missed class. Schedule your make up at the front desk. You can make up classes at the same level or lower.
37. The studio sends emails and texts frequently. We send a weekly competition newsletter (The Wednesday Word) and a studio wide monthly newsletter too. Read all the information we send out. For example, there may be additional information in the studio newsletter that was not in the Wednesday Word. Please make sure we have the best contact information on file. Let us know if you want your dancer to receive the newsletters as well.
38. 37) During costume measurements week, if you do not want us to measure your dancer without you being present, please email the front desk at help@dancedynamicslv.com. Also, when costumes come in, we typically have dancers try their costumes on in the studio. If you are more comfortable, send your dancer to classes this week in form fitting attire so we can try their costume on over their clothes.
39. The Lounge area at the back of the studio is for Dancers only. Parents do not sit there.
