REGISTRATION FEE
Please note there is a $25 yearly registration fee due at the time of registration that is non-refundable. ***Note this does not apply to Summer Season classes***
TUITION
I will pay tuition on or before the 10th of each month. Tuition is due the 1st and is considered late after the 10th. Payments may be made online if using a credit card. Cash or checks may be dropped off at the studio. (Please make checks payable to Center Stage Dance Studio). I will be billed only for the classes that my child or I am enrolled in.
TUITION LATE FEE
I will be charged a $3 late fee PER DAY, including weekends, for all tuition that arrives after the 10th of each month.
RETURNED CHECK FEE
I will be charged a $25 fee for returned checks.
REMOVAL FROM CLASSES
If tuition is more than 30 days late, my child or I will not be allowed to participate in class until it is paid in full. Also, if tuition is not paid in full by time of showcase my child or I will not be able to participate in the showcase.
REFUNDS
Refunds will be issued only due to a medical condition. A 20% processing fee will be deducted from the refund. For students who enroll late in the month, tuition will be reduced by no more than two classes.
MAKE-UPS
Up to 4 missed classes can be made up until the end of each session, August-December and January-June by attending another CSDS class of the same level. No credits or refunds will be given on classes missed due to illness, school holidays, inclement weather or other activities. Excessive absences will result in your child or you not being able to participate in our showcase in June.
PERFORMANCE DATE
Our anticipated performance date is June 28,2025. Please check your calendars and make sure there are no confilcts. You or your child are not required to perform in the showcase. If you choose to, you or your child must be at all Tech and Dress Rehearsals the week before the show.
ABSENT FOR SHOWCASE
If my child or I are absent from more than 3 classes in a month or any unexcused absences three weeks prior to performances, my child or I may be removed from performing.
WITHDRAWING FROM CLASS(ES)
I must give Center Stage Dance Studio 30- day written notice if my child needs to drop out of classes taken at CSDS. (Temporary absences due to injury, illness, family emergency, etc may be discussed with the directors. Please email the studio.
PHOTO/IMAGE RELEASE
I give my consent for images (photographs, video) of my dependent or myself to be taken and used to document the activities of Center Stage Dance Studio. These may be used on all social media such as, Instagram, Facebook, YouTube, etc. I grant Center Stage Dance Studio permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent or myself to be used, I will indicate this in writing and the signed letter will be attached to this document.
LIABILITY WAIVER
I acknowledge that I am the parent/guardian with legal responsibility for the above-referenced participant(s) and I am authorized to legally bind him/her. I also acknowledge that any program provided by Center Stage Dance Studio, 121 Joerschke Dr, Grass Valley, CA 95945 involves movement and motion and may result in physical injury. I also acknowledge that dance is an art form that requires teachers and instructors to be able to have appropriate physical contact for the purpose of making technical corrections. With this understanding and in consideration for the provision of Center Stage Dance Studio programs and services, I permit my child/children to participate in Center Stage Dance Studio programs and, on behalf of myself and/or my participating children.
RISK
Agree to assume all risk, bear all responsibility, to indemnify, hold harmless and covenant not to sue Center Stage Dance Studio, its agents, representatives, officers, and employees, from or for any and all claims (including, but not limited to claims of negligence or carelessness), causes of action, suits, proceedings, liabilities, judgments, awards, losses (including, but not limited to, loss of personal property) and damages, including costs, expenses and attorney fees related thereto, arising out of, resulting from or in any way related to or connected with (i) my/his/her participation in or attendance at Center Stage Dance Studio programs (no matter where such programs are located), and (ii) occurrences at or around Center Stage Dance Studio property, facilities or programs.
RELEASE
Forever release and discharge Center Stage Dance Studio, its agents, representatives, officers, and employees, from and against any and all claims (including, but not limited to claims of negligence or carelessness), causes of action, suits, proceedings, liabilities, judgments, awards, losses (including, but not limited to, loss of personal property) and damages, including costs, expenses and attorney fees related thereto, arising out of, resulting from or in any way related to or connected with (i) my/his/her participation in or attendance at Center Stage Dance Studio programs (no matter where such programs are located), and (ii) occurrences at or around Center Stage Dance Studio property, facilities or programs.
EMERGENCY
In case of an emergency Center Stage Dance Studio staff will call 911
Dress Code/Dance Attire
Pre- Dance & Combo Classes- Girls: Leotard, tights, pink ballet slippers and tap or jazz shoe, hair pulled back in a bun.
Skirts are okay. Boy: Black legging/Dance pant, white t-shirt.
Ballet – Girls: Leotard , tights, pink ballet slippers, hair pulled back in a bun (warmups and skirts may be worn with
instructors’ approval). Boy: Black legging/Dance pant, white t-shirt.
Pointe/Pre-Pointe – Leotard and tights. Footwear may be ballet flats, or pointe shoes (pointe shoes may only be worn with
instructors’ approval). Hair pulled back in a bun.
Jazz, Contemporary & Modern– Girls: Leotard of any color, fitted bottom (black tights, leggings, or jazz pants)
Boys: Comfortable easy to move in clothing. For footwear jazz shoes, socks, or paws (check with the instructor for their
preference). Hair pulled back.
Tap – Comfortable easy to move in clothing and tap shoes. Hair pulled back.
Tumbling– Leotard (girls do cartwheels and moves that take them upside down), athletic shorts or leggings, no shoes. This
is a safety requirement. Hair pulled back.
Hip Hop & other non ballet or jazz/tap classes – Loose-fitting clothing that is easy to move in. For footwear socks or tennis
shoes that have not been worn outside. Check with the instructor for more details.
**Mid-drifts are never allowed to be shown.
Studio Policies:
**Parent Waiting Areas**: We kindly ask that parents do not enter classrooms or gather near the classroom doors, benches in the hallways, or cubby areas. Teachers and staff will usher your dancers to you in the lobby. Please do not collect them from the dance rooms. We have a designated front lobby space for you to wait comfortably. You can view the classes through the windows outside. The space near the classroom doors is essential for students transitioning between classes. Please help us keep this area clear so students can prepare and move between activities smoothly. You are welcome to run errands during your child's class time. You are not required to stay at the studio. Please let us know if you are going to be more than 5 minutes late picking them up.
**Drop-Off**: Dancers may be dropped off no more than 5 minutes before their class start time. If your dancer has longer than a 15 min break between classes they must wait with you. This helps us manage transitions and keep everyone safe. **Dancers taking classes in Room 6. You will enter the building at the far end of the parking lot. There is no access through the lobby. Parents are not permitted to enter this room as there is no lobby space. Please wait in your car with your dancer until the classroom doors open. There are windows to view outside.
**Attire**: Please ensure that your dancer is dressed appropriately for their classes and that their hair is pulled back. Proper dance attire allows for optimal movement and safety. Attire by class is on our website. Buns are required for ballet classes.
**Personal Items**: Please be sure that your dancer has a small dance bag where they can keep their dance shoes and water bottles. Make sure that you have labeled their shoes with their name on the inside. Please label any water bottles with their name. This will help us get your dancer's item back to them. We do have a lost and found so check it often for missing items.
**Make-Up Classes**: Students are allowed to make classes they have missed due to illness. They may take a class in their same age group. Please email the studio first to check availability. We ask that classes are made up during the same month if possible. There are no make-ups for scheduled Holiday and break closures.
**Speaking with Teachers**: We ask that if you have a concern that you would like to discuss with a teacher that you email the studio. Teachers have back to back classes and need to be attentive to the students they have. The staff at the front desk can usually answer most questions but you can always email also.
**Tuition**: Tuition is due on the 1st of every month, late on the 10th. If you are paying by credit card you can log into your Pro Studio account and pay your bill there directly. If you would like to pay by check or cash you may drop it off at the studio. Students using Charter home school funds please email us and let us know what school we will be billing and for some additional information about payments and policies regarding this process.
