We require a $25.00 + GST, non-refundable insurance & registration fee per dancer. I accept that the deposit is non-refundable.
All Dance fees are to be paid in advance, by term in September and February. Studio-related charges will appear as REISHDA on your statements.
There is a $10 fee for any payments processed after the 5th of each month.
We require recurring Credit Card payments or eTransfers. There will be a $45 (non-negotiable) fee for all returned account closed or NSF cheques. You may choose to pay the entire tuition for your family in 1 lump sum.
We require 30 days' written notice should you wish to terminate your dance contract. The 30 days begins when we receive the letter which will be date stamped at our office.
In the event of mandated/unscheduled closures (COVID-19 or other) make up classes, if required, may be offered online and refunds will not be given.

I agree to be bound by the information in the Silver-Hope Dance Academy Student & Parent Handbook.
I acknowledge that there is a risk of injury involved in dance and fitness classes. I accept the risk and release Richardson Enterprises Inc. - SHDA (O/A Silver-Hope Dance Academy) from all liability.
I consent to Silver-Hope Dance Academy using photos/images of my child(ren) and myself if I am taking classes for promotional use at any time.

Silver-Hope Dance Academy Health & Safety Guidelines and Waiver

I agree to ensure that my dancer(s), family, and anyone I am living with adheres to any and all studio and government health guidelines that are in place. I recognize that the guidelines may change and I agree to adhere to changes as they occur.

I will not bring my child(ren) to class if they exhibit any of the following symptoms:
coughing, sneezing, fever (temperature above 37.2 degrees C/99F), chills, shortness of breath, sore throat, painful swallowing, stuffy or runny nose, loss of sense of smell, headache, muscle aches, fatigue, loss of appetite, body or stomach aches, chest congestion, diarrhea, or vomiting.

I will alert the Studio Owner/Director if my child is ill so that they can contact their peers, teachers, etc. as/if required.

I recognize that Richardson Enterprises Inc. – SHDA (O/A Silver-Hope Dance Academy) takes every precaution to ensure everyone’s health and safety, but they cannot guarantee it. I will do my part by following guidelines and safety protocols to help ensure everyone’s health and well-being.

I release Richardson Enterprises Inc. – SHDA, its contractors, owners, and assigns of any and all liability related to any illness or injury that may occur as result of and related to my child(ren)’s participation in class(es).
