I, the parent or legal guardian of the student(s) listed above, hereby give approval of the student(s)’s participation in any and all of the programs and activities, including, but not limited to the classes listed above. I waive, release, absolve, indemnify, & agree to hold harmless the organizers, sponsors, supervisors, participants and persons involved in the operation of these programs, activities, and classes for any claims arising out of injury or other loss to named applicant or any member of his/her family whether as a participant or spectator.

I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by The Studio and Studio K Dance.

I/we agree to release and hold harmless The Studio and Studio K Dance including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future.

I/we will not hold The Studio or Studio K Dance liable for any personal injury or any personal property damage,
which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by
The Studio and Studio K Dance.

I acknowledge and consent to allow The Studio and Studio K Dance to use:
photos and/or videos of my child/myself in publications (such as flyers/brochures),
advertisements (such as newspapers or online ads), our studio’s website, or on social
media sites (such as the studio’s facebook or instagram account). I understand that in
some cases, my child and/or my name may be used. I further understand no financial
compensation will be for use of these photos/videos.
