Registration deposit must accompany this form for a place in the class to be held. Monthly payments are due on the FIRST lesson of each month.
TUITION STATEMENTS WILL BE EMAILED IF PAYMENTS HAVE NOT BEEN RECEIVED IN OUR OFFICE BY THE 10TH OF EACH MONTH. A late fee of $10.00 will be assessed if payments are not received in our office by the 10th of each month. A late fee of $10.00 will be assessed to any charges not paid by the due date (costume deposits, costume balances, recital fee, etc.) A $25 fee will be assessed on all NSF checks.
Classes run continuously from September through May. Holidays are listed in your registration folder in the calendar section. You are responsible for nine full months of tuition unless a notice of withdrawal is given. Payments are monthly unless you are using our pre-payment plan at which time discounts of 5% will be deducted if you choose to pay the entire nine months in full. The full tuition payment does not include the registration fee, costume deposit and balance, and recital fee. Please be advised that all payments are non-refundable! NO REFUNDS ARE GIVEN FOR CLASSES NOT ATTENDED BUT MAKEUP CLASSES MAY BE SCHEDULED WITH THE FRONT OFFICE WITHIN FOUR WEEKS OF THE MISSED CLASS. ALL FEES ARE NON-REFUNDABLE. NO MAKEUP CLASSES ARE ALLOWED DURING THE MONTHS OF JANUARY THROUGH MAY DUE TO RECITAL DANCES.
The undersigned acknowledges and fully understands that he/she uses the facilities of Premier Dance Center LLC. at his/her own risk. The undersigned also acknowledges that the participant has medical insurance and hereby understands that Premier Dance Center carries liability as secondary coverage only. The undersigned hereby releases and forever discharges Premier Dance Center LLC., its agents, employees, independent contractors, officers, and directors from each and every claim, demand, or other obligation which may arise out of the connected activity. Premier Dance Center LLC. maintains reasonable rules of safety with all children, but I realize that due to physical demands and nature of the activity, there is a risk of injury. I am signing this document on behalf of myself and my child.
By signing, I understand all of Premier Dance Center’s policies and I agree to pay nine months of tuition unless a notice of withdrawal is given. I also understand I may be charged a fee for any NSF checks I write.
Students of Premier Dance Center, LLC. may have the opportunity to be photographed or video taped at various dance events or classes. With permission from parents/guardians these photos or video may be included for Premier Dance Center, LLC. publications or advertisements, including on our website and various social media sites. To ensure privacy, last names of children will not be included. No financial compensation shall be given for use of any photographs or video.
I acknowledge that Premier Dance Center, LLC. may choose not to use my student’s photos or videos at this time, but may do so at its own discretion at a later date. Premier Dance Center, LLC. reserves the right to discontinue use of photos or videos without notice. I have read this release and I understand its contents. I consent to and authorize the use by Premier Dance Center, LLC., or anyone authorized by Premier Dance Center, LLC., of any and all photographs or video which have been taken of my child/children.
