We, the staff of Footprints Dance Alliance, recognize our obligations to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver, you release Footprints Dance Alliance, owner Bonnie Dixon and her family and all her employees/sub-contractors from any liability now or in the future including by not limited to heart attacks, muscle strains, pulls, tears, broken bones, shin splints, heat prostration, knee, lower back, or foot injuries and any other injury, sickness or death however caused, as well as for expenses due to loss or damage to personal property which may be sustained by your child while participating in any Footprints Dance Alliance program at any time, while in the vicinity of the premises of the above stated business, or in any activity sponsored, represented or organized by Footprints Dance Alliance, owner Bonnie Dixon and her family, or employee/sub-contractors for any reason. You also affirm you now have, and will continue to carry, proper primary medical, health, and hospitalization and accident insurance, which you consider adequate for the protection of both your child and Footprints Dance Alliance.
____Payment of tuition is due by the 1st of the month and late by the 10th. A late fee of $25 will be
charged to any account not paid by the 10th. We do not send monthly statements, however you will be notified of any unpaid balances on your account.
_____A 30 day written notice is required upon withdrawal. Without proper written notice, your account will continue to be billed and you will be responsible for those payments. All tuition and fees paid prior to withdrawal are non refundable.
____A $30 returned check fee will be added on all insufficient fund checks and a $30 credit card rejection fee.
_____If your child is absent due to any reason: illness, vacation or other activities, payment is STILL due by the 10th. There are NO REFUNDS for missed classes.
_____If parents are splitting payment for their dancer(s), written or spoken consent must be given from BOTH parents to whom is paying what portion of the charges. Any changes in this agreement must be updated at the front desk.
_____Tuition remains the same each month regardless of whether it is a 5-week month or a shortened month due to Holidays. Tuition is not pro-rated, refunded or credited for missed classes or holidays. Any student who misses a class may make up classes in a comparable age/skill leveled class.
____All accounts must be completely paid off by May 1st for dancers to be able to participate in the end-of-the year recital.
_____Photo Release: by initialing here, I give permission for photographs of my child in dance class, performances or Footprints Dance Alliance related activities to be used in promotional material for Footprints Dance Alliance in both print and web publications.
____I fully understand that the use of alcohol, tobaccos, illegal drugs and/or demonstration of unacceptable standards of behavior will result in the dismissal of my child from the studio with no tuition refund.
_____We reserve the right to combine, cancel, change time, substitute or replace a teacher on any class.
______At Footprints Dance Alliance, end-of-the year recital is optional. I understand that commitment to the performance or declining the opportunity to perform must be made by November 10th .
_____I understand that my child must be picked up immediately following the end of their class. If my child is left unattended for 30 minutes, we will have to notify the authorities.
