Participation Waiver and Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

I, the undersigned, understand that by the very nature of dance, my child is subject to serious injury, illness and even death. I hereby release Anderson-Hoxie Dance Project, its staff and all
its heirs from any and all claims of damage, injury and illness as a result of participation of the above-named minor in any and all events, classes or activities offered by Anderson-Hoxie
Dance Project.

I, the undersigned, understand that I am liable for all applicable fees and charges due in exchange for dance classes, apparel and events including classes utilizing Zoom in the case
we are unable to hold in person classes due to Covid19. I understand that refunds will only be given for cases of withdrawal due to injury or illness verifiable by a physician, or in cases of
withdrawal or cancellation with notice. I understand that one month’s prior notice is required prior to withdrawal from class.

I, the undersigned understand and agree to release Anderson-Hoxie Dance Project from any and all claims of damage or theft of any personal property used or left on the premises or any
locations in use by the Anderson-Hoxie Dance Project for related events or activities.

I, the undersigned understand and agree that any and all photographs and video footage of my child obtained during classes or events may be used in publications related to the
Anderson-Hoxie Dance Project without further notice or compensation.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of
groups of people.

Anderson Hoxie Dance Project (AHDP) from has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you or your child(ren) will not become infected
with COVID-19. Further, attending classes at AHDP could increase your risk and your child(ren)’s risk of contracting COVID- 19.

Please feel free to consult with your lawyer regarding this document which may affect your legal rights. By signing this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily
assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending classes at AHDP and that such exposure or infection may result in personal injury, illness, permanent
disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 at the AHDP may result from the actions, omissions, or negligence of myself and others, including, but not limited to, AHDP
employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself, including, (but not limited to, personal injury, disability, and death), illness,
damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance in AHDP classes.

On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Anderson Hoxie Dance Project, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of AHDP, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any AHDP program.
