I hereby agree to the following:
It is understood that each registered student is, upon receipt of a $15.00 registration fee, enrolled in Dance Artistry Unlimited, Inc., for the ten-month school term September 2024 through June 2025 or date of enrollment through June 2025.
With the electronic signing/acknowledgement of this form I agree to pay for all lessons in advance, on the 1st of each month and to include a $15.00 late payment fee for any payments received after 7:30pm on the 5th of each month. There is a $35.00 fee for each returned check. No bill can be carried over 45 days. Monthly tuition payments run from the date of enrollment through and including the month of June and are never prorated unless I choose to pay on a per-lesson basis at a rate of $20.00 per class hour ($15.00 per 30 or $18 per 40 or 45 min class) or purchase a dance punch card at the rate provided on the card.
In accordance with the website, brochure and pre-registration agreement, it is understood that tuition is non-refundable. There shall be no discount or refund for holidays, school closings, weather cancellations, or lessons missed, and tuition is due in full for the ten-month instructional period mentioned above. Students who choose to make up a missed class must do so within 14 days, or the make-up class will be forfeited. Due to the low monthly rate, make up classes are only offered if weather cancellations exceed six per season. Typically, DAUI schedules a minimum of thirty-six (36) lessons per season at each level, which exceeds the minimum thirty (30) lesson requirement.
It is to be noted that students are enrolled for one, ten-month, school year unless I agree to an hourly payment plan at a rate of $20.00 per hour ($15.00/30; $18.00/40 or 45-min class) for each session of instruction, payable at the opening of each lesson. As such, all tuition agreed upon in this contract must be paid through and including June unless a written, 30-day advanced notification of withdrawal is received and documented by the owner. The parent/guardian and/or student is liable for payment in full through June unless the director agrees to release you from this obligation due to long-term illness, injury, or relocation beyond the studio area (50+ miles). Requests for release due to severe illness/injury or relocation must be confirmed by a written recommendation for withdrawal by the student's physician or a letter of military move/deployment or civilian location reassignment. If the registered party is released from this 10-month contractual obligation for one of the aforementioned reasons they will be billed for any outstanding tuition, costume balances, late fees, etc., plus one additional month's tuition. Without written and signed notice from you and receipt of written confirmation from the director acknowledging said withdrawal from your original 10th month contract for extenuating circumstances (written, dated, and signed by the director) you will remain accountable for tuition, fees and costume payments through June as stated herein and all related collection and legal fees necessary to obtain all funds due should you default on this contract.
This agreement is legal and binding as written except in the event a parent/guardian/student submits a written request for a change in class hours. Any increase in hours is effective immediately and receipt of funds will serve as proof of consent to a change in the contractual hours and payment amount of this contract. Any decrease in hours requires a written request on or before the first of the month plus one additional month's tuition at the amount agreed to in this contract, before any tuition reduction will take effect. This agreement also requires a form that must be signed by the director to acknowledge the change in classes. No dancer may drop more than one class per season - this excludes dance company membership, where the dancer and parents must adhere to the guidelines of the DASL company contract.
Dance Artistry Unlimited, Inc. is obligated to conduct classes as scheduled providing qualified staff and apprentice teachers and to offer make up classes or an alternate instructor in the case of an illness, conflict, or family emergency on the part of a staff member. Schedules are subject to change if it should be deemed beneficial to the majority of the class or should there be an enrollment level five students or less.
I, parent and/or guardian of the registered dancer(s), hereby agree to hold Dance Artistry Unlimited, Inc., its owner, the owner's employees or family, its agents, and all sponsors, free from any and all liability, claims, judgments, or demands for any injury to person or loss/damage to property sustained by the undersigned or student or any person claiming through the undersigned or student, resulting from any accident or occurrence on the premises, at a DAUI/DASL sanctioned event, or as a result of any course of instruction given the applicant by Dance Artistry Unlimited, Inc. In addition, I do agree to incur and provide payment or compensation for any and all damages the applicant or any family member or acquaintance accompanying the applicant may cause while on the premises or at any studio-related function. I also understand and consent that any photos taken by/for Dance Artistry Unlimited, Inc. may be used for advertising, social media, and/or publicity purposes for the duration of the studio's existence, but will not, in any way, reveal my child's full last name, age or address.
