Please Make sure and AGREE that you are 18 years or above to register for an account and that you will AGREE to all rules and policies set forth by Studio 8 performing arts. I AGREE to all of Studio 8 performing arts Policies and Guidelines set forth while myself/child are enrolled in dance school from this day forward. I promise to read and follow the rules and regulations of the Policies and Guidelines that will be sent in a Welcome Packet via email upon registration in this portal, posted in the files section of this parent portal website, on the Facebook parent group page, and on our studio website via the internet. Also, upon registering myself/child with Studio 8 performing arts I agree and will adhere to the Insurance Policies set forth by Studio 8 performing arts. I agree that Studio 8 performing arts will not be liable and will be released for any and all accidents and or injuries while attending, dancing in or with the studio on or off premises.

Cancellation, Guidelines & Photo Policy

I agree when registering myself, my child, my grandchild online through Studio 8 Performing Arts online dance studio software that I will give a two week notice prior to the 1st of the following month for any cancellations or I will be responsible for paying the following month plus a $25 cancellation fee. After the final month has been paid in full including all outstanding balances, I understand that my credit card on file will be deleted and not charged through Studio 8 Performing Arts.

I agree in all of Studio 8 Performing Arts Policies & Guidelines set forth in the files menu of this site when I register my dancer to take classes in the 2023-2024 Summer/Fall/Spring Seasons. I understand that I will read in full the entire packet and sign and turn in page of the Policies and Guidelines Handbook upon my dancers first dance class and sign waiver upon registration.

I agree to allow my dancers pictures and or videos taken by Studio 8 Performing Arts to be used through social media, advertisements or any other source of marketing for the studio. I sign off on all pictures and videos made to Studio 8 performing arts.
