THIS CONTRACT IS SUBJECT TO THE TERMS, CONDITIONS, AND POLICIES STATED HERE
 Payments are due the first of the billing month, payable by credit card via Autopay. On the 10th of each month, a $10 late fee will be charged if payment has not been made due to invalid credit card numbers, or insufficient funds. For accounts in arrears, after 60 days the student will be asked to withdraw from classes. A $25 fee will be charged for returned checks. 
A one-time Registration Fee of $20 per student or $30 per family is due at time of registration. This fee is non refundable. Classes are not prorated. Tuition is the same every month, regardless of holidays and/or student absences. Students may attend another class to make up an absence. Students agree to follow the dress code, attend practices regularly, treat teachers and fellow dancers respectfully, and not have food or gum in class. 

 Spark Dance Academy is not responsible for children left before or after class. Prompt drop-off and pick-up is required. Students not picked up within 15min of class ending will incur a $10 fee to be drafted with tuition. Spark Dance Academy is not responsible for personal property. The Academy cannot be held responsible for changes in instructors & schedules. Classes must have consistent attendance of 3 or more students to be retained in the schedule. It is highly encouraged that dancers participate in our spring concert, a participation and costume fee of $65 for each costume. This fee will be drafted through Auto-pay on October 1st for all classes. Please notify us BEFORE this date if your student will not be performing in the Recital. This fee is non refundable.  Ticket prices for all performances are announced by March 1, and the necessary forms and instructions are emailed. Students are not allowed to perform until any outstanding balance for tuition is paid in full.

Students agree 
 In attending Spark Dance Academy, taking dance or exercise classes and otherwise using the facilities, equipment therein, or participating with outside the studio, I do so at my own risk. Spark Dance Academy shall not be liable for any damages arising from personal injuries incurred by me in, on or about the premises of the Spark Dance Academy dance studio/program, including performances outside the studio. I assume full responsibility for any injuries or damages which may occur to me in, on or about Spark Dance Academy studio/program, and I do hereby fully and forever release and discharge Spark Dance Academy, its directors, officers, dance instructors, employees and agents from any and all claims, demands, damages, rights of action or causes of action, present or future, whether the same be known, anticipated or unanticipated, resulting from or arising out of my use of said Spark Dance Academy dance studio/program, dance classes, performances, facilities and equipment thereof.
Spark Dance Academy photographs and videotapes its students in class and performance, and occasionally uses these photographs and videotapes in its advertising and promotion. By signing this contract and enrolling your child at the Academy, you agree that your child’s picture/video may be taken and that you consent to the use of these pictures and videos by the Academy for promotional purposes

Classes must have consistent attendance of 4 or more students to be retained in the schedule. It is highly encouraged that dancers participate in our spring recital. There is a costume fee that will be automatically charged to the card on file on October 1st. This fee is non-refundable, If you want to opt-out of the recital/costume fee you must let us know before October 1st.

COVID-19

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Spark Dance Academy LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.I further acknowledge that Spark can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others and release Spark Dance Academy from any and all liability for unintentional exposure or harm due to COVID. Spark Dance Academy photographs and videotapes its students in class and performance, and occasionally uses these photographs and videotapes in its advertising and promotion. By signing this contract and enrolling your child at the Academy, you agree that your child’s picture/video may be taken and that you consent to the use of these pictures and videos by the Academy for promotional purposes.
